乳腺钼靶X线摄影检查联合磁共振成像诊断乳腺导管原位癌的有效性

The value of mammography combined with magnetic resonance imaging in the diagnosis of ductal carcinoma in situ

目的探究应用磁共振成像(magnetic resonance image,MRI)联合乳腺钼靶X线摄影检查诊断乳腺导管原位癌(ductal carcinoma in situ,DCIS)的临床价值。方法选择2014年10月至2016年8月深圳市龙华区中心医院确诊的105例DCIS患者,对其分别行MRI检查和乳腺钼靶X线摄影检查,后经手术病理确诊,观察MRI检查联合乳腺钼靶X线摄影和MRI单一诊断乳腺DCIS的临床价值与满意度。结果 105例乳腺DCIS患者应用MRI联合乳腺钼靶X线摄影检查的敏感度、特异度和准确度分别为91.38%、89.36%和90.48%,明显优于采用单一MRI诊断(57.41%、47.06%和52.38%),差异有统计学意义(P<0.05)。结论针对乳腺DCIS患者选择应用MRI联合乳腺钼靶X线摄影检查可提高诊断效能,有利于疾病的早期诊治。

Objective To explore the clinical value and characteristics of mammography combined with magnetic resonance imaging（MRI） in patients with breast ductal carcinoma in situ（DCIS）. Methods 105 patients with DCIS in Central Hospital of Longhua District from October 2014 to August 2016 were selected. MRI and mammography were performed on them respectively. Surgical pathology confirmed the diagnosis. To observe the clinical value and satisfaction of MRI examination combined with mammography and MRI alone in the diagnosis of breast DCIS. Results 105 cases with DCIS were examed hy MRI combined with molybdenum target mammography for its sensitivity, specificticy and accuracy. The results were 91.38 %, 89. 36 % and 90. 48 %, respectly, and were significantly better than 57.41%, 47.06 % and 52. 38 % of the MRI alone. The differences were statistically significant （P 〈 0. 05）. Conclusion MRI combined with molybdenum target mammography in diagnosed of the patients with DCIS can improve the diagnostic accuracy. It is benefit to the early discovery and diagnosis of the disease, and can provide help and reference for later targeted therapy.
