摘要
目的探讨拉萨藏族社区非高脂血症人群同型半胱氨酸(Hcy)与总胆固醇(TC)和三酰甘油(TG)的关系。方法 2016年采集来自拉萨城关社区非高脂血症人群的各项资料。采用多因素线性回归分析Hcy对TC和TG的影响。结果研究对象共1054人,男性352名(33.40%)。TC均值为(3.90±0.85)mmol/L,TG和Hcy的中位数(P25-P75)分别为0.91 mmol/L(0.69~1.18 mmol/L)和14.6μmol/L(13.2~16.4μmol/L)。调整性别、年龄、体重指数等危险因素后,Hcy每升高5μmol/L,TC和TG分别增加0.08 mmol/L(95%CI 0.03~0.13)和0.04 mmol/L(95%CI0.02~0.05)(P均<0.001);与Hcy<10μmol/L组比较,Hcy在(10~15)μmol/L组的TC和TG分别增加1.07 mmol/L(95%CI 0.72~1.42)和0.26 mmol/L(95%CI 0.13~0.40),Hcy>15μmol/L组的TC和TG分别增加1.11 mmol/L(95%CI0.76~1.46)和0.28 mmol/L(95%CI 0.15~0.42)(P均<0.001)。结论在拉萨城关区非高脂血症藏族人群中,血清Hcy影响TC及TG水平,提示Hcy可能参与血脂代谢过程。
Objective To investigate the association of both total cholesterol (TC) and triglyceride (TG) with homocysteine(Hcy) in a community-based Tibetan population without hyperlipidemia in Lhasa. Methods A total of 1054 Tibetan subjects without hyperlipidemia were included in 2016. Multivariate linear regression analyses were used to examine the effect of Hcy on TC and TG. Results Among the 1054 people, about 33.40% (n=352)ofthe participants were man. The average TC level was (3.90+0.85) mmol/L, and median (P25-P75) of TG and Hcy were 0.91mmol/L (0.69-1.18 mmol/L) and 14.6 μmol/L(13.2-16.4μmol/L), respectively. After adjustment for sex, age and risk factors, such as body mass index, an average increase of 5 umol Hcy was associated with an increase of 0.08 mmol/L (95%ci 0.03-0.13) of TC and 0.04 mmol/L(95%CI 0.02-0.05) of TG (all the P〈0.001). TC and TG in second group (15μmol/L〉Hcy ≥10μol/L) (β for TC: 1.07; β for TG: 0.26) and third group (Hcy ≥15μmol/L) (β for TC 1.11; β for TG 0.28) were higher than first group (Hcy 〈10μmol/L), all the P〈0.001(95%CI was respectively: 0.72-1.42, 0.13-0.40, 0.76-1.46, 0.15-0.42). Conclusion Hcy has effect on TC and TG level in the community-based population in Lasa without hyperlipidemia, which indicates that Hcy may participate in the metabolism of lipid.
