氟比洛芬酯预先给药对腹部手术老年患者肠系膜牵拉综合征的影响

Effect of flurbiprofen axetU pretreatment on mesenteric traction syndrome in elderly patients undergoing abdominal surgery

目的评价氟比洛芬酯预先给药对腹部手术老年患者肠系膜牵拉综合征的影响。 方法择期全麻下行开腹胃肠道手术患者60例，性别不限，年龄65～86岁，体重指数18.8～25.6 kg/m2，ASA分级Ⅰ或Ⅱ级。采用随机数字表法分为2组（n＝30）：氟比洛芬酯组（F组）和生理盐水组（NS组）。F组麻醉诱导时静脉注射氟比洛芬酯50 mg，NS组静脉注射等容量生理盐水。分别于麻醉诱导即刻、肠系膜牵拉即刻、肠系膜牵拉后10、15和30 min时，采集动脉血样，采用ELISA法检测血浆6-酮-前列腺素F1α（6-keto-PGF1α）浓度。记录牵拉肠系膜后30 min内肠系膜牵拉综合征的发生情况。 结果与NS组比较，F组牵拉肠系膜后各时点血浆6-keto-PGF1α浓度降低，牵拉肠系膜后各时段肠系膜牵拉综合征发生率低（P〈0.05）。 结论氟比洛芬酯预先给药可有效地预防老年患者肠系膜牵拉综合征的发生。

Objective To evaluate the effect of flurbiprofen axetil pretreatment on mesenteric trac- tion syndrome in elderly patients undergoing abdominal surgery. Methods Sixty patients of both sexes, aged 65-86 yr, with body mass index of 18.8-25.6 kg/m2 , of American Society of Anesthesiologists phys- ical status I or ]I , scheduled for elective gastrointestinal tract surgery under general anesthesia, were di- vided into 2 groups （n = 30 each） using a random number table： flurbiprofen axetil group （group F） and normal saline group （ group NS）. Flurbiprofen axetil 50 mg was intravenously injected during anesthesia in- duction in group F, while the equal volume of normal saline was given instead in group NS. Immediately af- ter anesthesia induction, immediately after mesenterie traction and at 10, 15 and 30 rain after mesenteric traction, arterial blood samples were collected for measurement of plasma 6-keto-prostaglandin FIa concen- trations using enzyme-linked immunosorbent assay. The occurrence of mesenterie traction syndrome was re- corded within 30 min after mesenteric traction. Results Compared with group NS, the plasma 6-keto-pros- taglandin FI~ concentrations were significantly decreased at each time point, and the incidence of mesen- teric traction syndrome was decreased during each period after mesenteric traction in group F （P〈O. 05）. Conclusion Flurbiprofen axetil pretreatment can effectively prevent the development of mesenteric traction syndrome in elderly patients undergoing abdominal surgery.
