基质金属蛋白酶-1基因-519A/G多态性与冠心病发病风险的Meta分析 被引量:1

摘要 目的采用Meta分析的方法探讨基质金属蛋白酶-1(MMP-1)基因-519A/G多态性与冠心病发病风险。方法计算机检索Pub Med、EMbase、CBM、CNKI、Wan Fang Data、VIP数据库,查找与主题相关的病例-对照研究,检索时限均为建库至2016年7月。两位作者独立进行文献筛选、资料提取及质量评价。然后采用Stata 12.0软件进行Meta分析。结果共纳入6个病例-对照研究,共计2027例患者,1654例对照。Meta分析结果显示,MMP-1基因-519A/G多态性在等位基因模型(G vs.A:OR=0.68,95%CI:0.49~0.94,P=0.02)、杂合子模型(AG vs.AA:OR=0.62,95%CI:0.40~0.95,P=0.03)、显性模型(GG+AG vs.AA:OR=0.61,95%CI:0.40~0.94,P=0.03)和隐性模型(GG vs.AA+AG:OR=0.76,95%CI:0.59~0.97,P=0.03)中降低冠心病发病风险,而在纯合子模型中无统计学意义(GG vs.AA:OR=0.59,95%CI:0.35~1.00,P=0.05)。亚组分析结果显示,降低冠心病的风险在亚洲人群中更加突出。结论当前证据表明,MMP-1基因-519A/G多态性可能会降低冠心病的发病风险,尤其是亚洲人群。由于纳入研究的数量限制,该结论尚需进一步开展研究加以验证。 Objective To discuss the relationship between polymorphism of gene-519A/G in matrix metalloproteinase-1 (MMP-1) and risk of coronary heart disease (CHD) by applying Meta-analysis. Methods The databases of PubMed, EMbase, CBM, CNKI, Wanfang Data and VIP Database were retrieved with computer for searching relevant case-control studies up to July, 2016. The literature screening, data extracting and quality reviewing were carried out independently by 2 authors. All data was given a Meta-analysis by using Stata 12.0 software. Results There were totally 6 case-control studies included involved 2027 patients and 1654 controls. The results of Meta-analysis showed that polymorphism of gene-519A/G in MMP-1 was related to the decrease of CHD risk in allelic genetic model (G vs. A OR=0.68, 95%CI: 0.49-0.94, P=0.02), heterozygote model (AG vs. AA: OR=0.62, 95%CI: 0.40-0.95, P=0.03), dominant model (GG+AG vs. AA: OR=0.61, 95%CI: 0.40-0.94, P=0.03), and recessive model (GG vs. AA+AG: OR=0.76, 95%CI: 0.59-0.97, P=0.03), while had no statistical significance in homozygote model (GG vs. AA: OR=0.59, 95%CI: 0.35-1.00, P=0.05). The results of subgroup analysis showed that the decrease of CHD risk was more significant in Asian population. Conclusion The current evidence suggests that polymorphism of gene-519 A/G in MMP-1 may be related to the decrease of CHD risk, especially in Asian population. Due to limited quantity of included studies, the conclusion needs further studies to be confirmed.
