血脂及血清相关炎性因子水平与妊娠期高血压疾病的相关性研究 被引量:33

摘要 目的观察血脂与子痫前期的相关性。方法选择2013年8月至2017年6月产科诊断妊娠期高血压、子痫前期、重度子痫前期孕妇各40例为研究对象,比较4组血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、对氧磷酶1(PON-1)、脂蛋白相关磷脂酶A2(Lp-PLA2)、脂联素(APN)浓度、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)水平变化。结果与正常对照组比较,妊娠高血压综合征组患者TC、TG、LDL-C水平升高,HDL-C水平降低,差异有统计学意义(P<0.05)。正常对照组、妊娠期高血压、子痫前期、重度子痫前期4组间TC、TG、LDL-C、HDL-C经过单因素方差分析后显示,4组间总体平均水平均各不相同(P<0.05)。4组间各项指标两两比较差异有统计学意义(P<0.05)。4组间血清炎性相关因子经过单因素方差分析后显示,各组间总体平均水平均各不相同(P<0.05)。CRP、IL-6、TNF-α、Lp-PLA2水平逐渐升高,PON-1、APN浓度逐渐下降。4组间各项指标两两比较差异也有统计学意义(P<0.05)。结论脂质代谢紊乱及血清炎症参与子痫前期发病机制,孕期密切监测孕妇血脂可诊断子痫前期的发病,预测疾病进展的程度。 Objective To investigate the correlation between blood lipids as well as serum inflammatory factors and preeclampsia. Methods Forty pregnant women with gestational hypertension,40 pregnant women with preeclampsia and 40 pregnant women with severe preeclampsia who were treated in our hospital from August 2013 to June 2017 were enrolled as subjects,at the same time,the other 40 healthy pregnant women were served as control group. The serum levels of total cholesterol( TC),triglyceride( LDL-C),high density lipoprotein( HDL-C),paraoxonase 1( PON-1),lipoprotein-related phospholipase A2( Lp-PLA2),adiponectin( IL-6),tumor necrosis factor-α( TNF-α) and C-reactive protein( CRP) were detected and compared among the four groups. Results As compared with those in control group,the serum levels of TC,TG,LDL-C and HDL-C in patients with gestational hypertension were significantly increased( P 〈0. 05). There were significant differences in the serum levels of TC,TG,LDL-C and HDL-C among the four groups( P 〈0. 05),moreover,there were significant differences in these indexes between two groups each other among the four groups( P 〈0. 05). In addition there were significant differences in the serum levels of inflammatory factors among the four groups( P 〈0. 05). The serum levels of CRP,IL-6,TNF-a and Lp-PLA2 were gradually increased,however,the levels of PON-1 and APN were gradually decreased,there were significant differences in these indexes between two groups each other among the four groups( P 〈0. 05).Conclusion The lipid metabolic disorder and serum inflammation are involved in the pathogenesis of preeclampsia,thus,to monitor closely the lipid levels of pregnant women can diagnose the incidence of preeclampsia and predict the progression of disease.
