糖尿病肾衰竭患者透析后应用三仙补肾方效果观察

Therapeutc effects of Sanxianbushen prescription on diabetic renal failure after dialysis

目的探讨糖尿病肾衰竭患者透析后应用三仙补肾方效果观察,旨在提高患者预后,延长生存周期,改善生活质量。方法将2015年10月至2016年10月98例糖尿病肾衰竭患者随机分为2组,每组49例。对照组采用维持血液透析加常规西医综合基础治疗,观察组在此基础上给予三仙补肾方治疗,比较2组患者临床效果、肾功能各指标情况、血糖血脂指标改善情况、中医症候改善情况、营养状况及不良反应。结果观察组显效24例、有效20例、无效5例,有效率89.80%,对照组显效16例、有效15例、无效18例,有效率63.27%,观察组治疗效果明显提高,2组差异有统计学意义(P<0.05);观察组治疗后尿素氮(BUN)、血肌酐(Scr)、肌酐清除率(Ccr)较对照组明显改善,2组差异有统计学意义(P<0.05);观察组治疗后空腹血糖(FBG)、餐后2 h血糖(2 hPG)、胆固醇(TC)、三酰甘油(TG)明显较对照组改善,2组差异有统计学意义(P<0.05);观察组治疗后倦怠乏力、气短懒言、头晕头痛、食少纳呆、自汗等症状积分明显低于对照组(P<0.05),但2组肌肤甲错、肢体麻木、便秘等症状积分差异无统计学意义(P>0.05);观察组治疗后红细胞(RBC)、血红蛋白(HGB)、血清总蛋白(TP)、血清白蛋白(ALB)水平较对照组明显提高,2组差异有统计学意义(P<0.05);2组治疗后复查血尿常规、肝肾功能、电解质等未见明显异常(P>0.05)。结论糖尿病肾衰患者透析后应用三仙补肾方效果显著,能有效缓解临床症状,促进血糖及肾功能各指标的改善,提升机体营养状况,且服用安全性高,具有积极的临床意义。

Objective To investigate the therapeutc effects of Sanxianbushen prescription on diabetic renal failure after dialysis in order to improve prognosis,prolong the life cycle and improve life quality of patients. Methods A total of 98 patients with diabetic renal failure who were treated in our hospital from October 2015 to October 2016 were randomly divided into two groups,control group and observation group,with 49 patients in each group. The patients in control group were treated by maintenance hemodialysis combined with routine western drugs,however,the patients in observation group,on the basis of control group,were treated by Sanxianbushen prescription. The clinical curative effects,renal function indexes,blood glucose levels,blood fat indexes,the improvement status of TCM syndromes,nutritional status and adverse reactions were observed and compared between two groups. Results There were 24 cases with obvious curative effects,20 cases with effectiveness,5 cases without effectiveness in observation group,with effective rate being 89. 80%,however,there were 16 cases with obvious curative effects,15 cases with effectiveness,18 cases without effectiveness in control group,with effective rate being 63. 27%,the curative effects in observation group were superior to those in control group（ P 〈0. 05）. After treatment,the urea nitrogen（ BUN）,blood creatinine（ Scr） and creatinine clearance rate（ Ccr） were significantly improved in observation grouo,as compared with those in control group（ P 〈0. 05）. Moreover the fasting blood glucose（ FBG）,2h blood glucose（ 2hPG）,cholesterol（ TC） and triglyceride（ TG） were significantly improved in observation group,as compared with those in control group（ P 〈0. 05）. The symptom scores of shortness of breath,lazy speech,dizziness headache,less diet,anorexia,spontaneous perspiration in observation group were significantly lower than those in control group（ P 〈0. 05）,but there were no significant differences in the symptom scores of squamous and dry skin,limbs numbness,constipation between two groups（ P 〈0. 05）. In addition the levels of red blood cells（ RBC）,hemoglobin（ HGB）,serum total protein（ TP）,serum albumin（ propagated） in observation group were significantly higher than those in control group（ P 〈0. 05）; After treatment,the blood and urine routine rest,liver and kidney function,electrolyte were not significantly changed in both groups（ P 〈0. 05）.Conclusion Sanxianbushen prescription has obvious therapeutic effects on diabetic renal failure after dialysis,which can effective relieve clinical symptoms,promote the improvement of blood glucose and renal function indexes,improve the body＇s nutritional status,with safety.
