单纯经腹部超声检查与联合经阴道超声检查对子宫腺肌病的诊断价值评价

Evaluation of diagnostic value of simple transabdominal ultrasonography and combined transvaginal ultrasonography in adenomyosis

目的研究分析单纯经腹部超声检查与联合经阴道超声检查对子宫腺肌病的诊断价值。方法选取2016年1月~2017年9月我院收治的100例经病理证实为子宫腺肌病的患者作为研究对象,利用计算机将其随机分为对照组与观察组,每组各50例。对照组采取单纯经腹部超声检查,观察组在此基础上联合经阴道超声检查。比较两组患者的诊断结果与病理结果。结果与病理结果比较,观察组中确诊46例、漏诊4例,诊断符合率为92.0%(46/50);对照组中确诊37例、漏诊13例、诊断符合率为74.0%(37/50),观察组与对照组比较,差异有统计学意义(P<0.05)。观察组测得子宫血流量、动脉搏动指数、动脉阻力指数等分别为(73.24±16.32)ml/min、1.24±0.12、2.85±0.82,对照组分别为(106.85±16.77)ml/min、0.73±0.16、1.13±0.82,观察组与对照组比较,差异有统计学意义(P<0.05)。结论联合经阴道超声检查对子宫腺肌病的诊断价值显著高于单纯经腹部超声检查,前者能提高诊断符合率,为疾病治疗提供可靠参考依据,值得临床应用推广。

Objective To study and analysis of simply by abdominal ultrasound examination and combined value of transvaginal ultrasonography in the diagnosis of adenomyosis.Methods 100 cases of patients with adenomyosis confirmed by pathology treated in our hospital from January 2016 to September 2017 were selected as the subjects,and randomly divided into control group and observation group by computers,50 cases in each group.The control group adopted simply by abdominal ultrasound examination,the observation group based on the combined transvaginal uhrasound.The diagnosis results and pathological results of the two groups were compared.Results Compared with the pathologic resuhs,46 cases were diagnosed in the observation group,4 cases of misdiagnosis,the diagnosis rate was 92.0% （46/50）.37 cases were diagnosed in the control group,13 cases of misdiagnosis,the diagnosis coincidence rate was 74% （37/50）,the observation group was significantly higher compared with the control group,the difference was statistically significant （P〈0.05）.The observation group measured blood flow to the uterus artery,pulsatility index,arterial resistance index （73.24±16.32） ml/min, 1.24±0.12,2.85±0.82,the control group were （106.85±16.77） ml/min,0.73±0.16,1.13±0.82, the observation group compared with the control group,the difference was statistically significant （P〈0.05）.Condusion The combination of transvaginal uhrasonography in diagnosis of adenomyosis is significantly higher than that of simply by abdominal ultrasound examination,the former can improve the diagnosis rate,so as to provide a reliable reference for the treatment of the disease, which is worthy of clinical application.
