强直性脊柱炎患者骨盆倾斜角与髋臼、股骨颈前倾角的相关性分析

The correlation analysis of the inclination of the pelvis to the acetabulum and the femoral neck obliquity in patients with ankylosing spondylitis

目的分析强直性脊柱炎患者骨盆倾斜角与髋臼、股骨颈前倾角的相关性。方法选取强直性脊柱炎患者30例为研究组,所有患者均接受人工髋关节置换术,保留所有患者的全部信息,所有患者签署研究知情同意书,另选取30例健康者为对照组,年龄及性别比例按照研究组标准选取。对全部60例进行影像学检查,包括X线、CT等影像学手段,测量股骨颈前倾角,骨盆倾斜角以及髋臼前倾角。结果经统计,研究组和对照组相比,骨盆倾斜角度与股骨颈前倾角差异均有显著性(P<0.05),说明强直性脊柱炎可以影响到骨盆倾斜角度与股骨颈前倾角的改变。强直性脊柱炎患者股骨颈前倾角增大,髋臼前倾角减小。将患者组的骨盆倾斜角与股骨颈的倾斜角做相关分析,结果显示两变量之间具有相关性,相关直线回归方程为:y=7.93+0.816x,P<0.05。将患者组的骨盆倾斜角与髋臼前倾角度做相关分析,结果显示两变量之间具有相关性,相关直线回归方程为:y=6.04-0.711x,P<0.05。结论强直性脊柱炎患者骨盆倾斜角与髋臼、股骨颈前倾角具有相关性,股骨颈的倾斜角随着骨盆倾斜角的增大而增大,髋臼前倾角随着骨盆倾斜角的增大而减小。

Objective To analyze the correlation between obliquity of pelvis and anteversion angle of acetabulum and femoral neck in patients with rigidity. Method In patients in our hospital bone surgery treated 30 cases as the study group, all patients underwent artificial hip replacement, to retain all the information of all the patients, all patients signed the informed consent of the other,30 healthy persons were selected as control group,age and sex ratio were selected according to the research. All 60 patients underwent radiographic examination, including X-ray, CT and other imaging methods to measure the anteversion of the femoral neck,the obliquity of the pelvis,and the anteversion angle of the acetabulum. Result to statistics,the research group compared with the control group,the pelvic tilt angle and femoral neck anteversion significantly,P〈0.05,indicating ankylosing spondylitis can affect the angle and femoral neck anteversion change of pelvic tilt. In patients with ankylosing spondylitis,the anteversion of the femoral neck increases and the anteversion angle decreases. Correlation analysis in patients with pelvic tilt will tilt angle and femoral neck angle, the result showed that there was correlation between two variables, the relevant regression equation ：y=7.930. 816 x,P〈0.05.The correlation between the tilt angle of the pelvis and the anteversion angle of the acetabulum was analyzed. The results showed that there was correlation between the two variables. The related linear regression equations were：y=6.04-0.711 x,P〈0.05.Conclusion Ankylosing spondylitis patients with pelvic tilt angle and acetabular and femoral neck anteversion with correlation, tilt the femoral neck with increasing pelvic tilt angle increases with the increase of acetabular anteversion, pelvic tilt angle decreases.
