经脂肪囊外途径后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿的疗效评价

Evaluation of the Therapeutic Effect of Laparoscopic Renal Pedicle Lymphatic Ligation in the Treatment of Chyluria

目的评价经脂肪囊外途径后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿的疗效。方法将2014年9月—2017年9月我院收治的26例乳糜尿患者作为研究对象,对26例乳糜尿患者均予以经脂肪囊外途径后腹腔镜肾蒂淋巴管结扎术治疗,分析及研究26例乳糜尿患者的治疗效果。结果 26例乳糜尿患者的平均手术耗时为(73.69±5.39)min,术中平均失血量为(84.65±6.54)ml,平均住院天数为(6.33±1.25)d。围术期没有出现其他的严重并发症。没有患者出现复发现象,治疗成功率为100.00%。结论在乳糜尿患者中采取经脂肪囊外途径后腹腔镜肾蒂淋巴管结扎术,与脂肪囊内途径比较,创伤更小、恢复更快、复发率低,疗效显著。

Objective To evaluate the curative efect of laparoscopic renal tubule ligation for the treatment of chyluria. Methods 26 cases of chyluria in our hospital from September 2014 to September 2017 were studied. 26 cases of chyluria were treated by ligation of renal pedicle lymphatic vessels via fat extracapsular approach. The therapeutic efect of 26 patients with chyluria was analyzed. Results The average operation time of 26 patients with chyluria was （73.69±5.39） min, the mean blood loss was （84.65±6.54） ml during the operation. The average number of hospitalization days was （6.33±1.25） d. There were no other serious complications during the perioperative period. No recurrence was found in the patients, and the rate of successful treatment was 100.00%. Conclusion Laparoscopic ligation of renal pedicle lymphatic vessels in the patients with chyluria after an extra fat capsule is performed. Compared with the approach in the fatty capsule, the trauma is smaller, the recovery is faster, the recurrence rate is low, and the curative efect is remarkable.
