我院2014–2016年辅助用药临床应用分析

Analysis of the utilization of adjuvant drugs in our hospital during 2014 – 2016

目的:了解我院辅助用药的规律和特点,为辅助用药合理使用和监管提供参考。方法:采用回顾性分析方法,对我院2014–2016年销售金额和用药频度(DDD_s)排名前10位的辅助用药的销售金额、用药频度、日均费用(DDC)、排序比(B/A)等进行统计分析。同时回顾"医院管理信息"公示的处方/病历点评,对辅助用药的相关内容进行统计。结果:2014–2016年,辅助用药销售金额从2 085.61万元增长至2 131.62万元,占药品总销售金额的比例从17.48%增长至18.95%;销售金额排名前10位的药品类别主要为活血化瘀类、自由基清除剂、神经营养类及其他类;DDD_s排名前10位的药品类别主要为活血化瘀类、神经营养类、自由基清除剂、肿瘤辅助用药和其他类;DDC较大以注射剂为主,且B/A同步性较差。辅助用药不合理使用排名前3位分别为自由基清除剂、其他类和活血化瘀类。结论:医院应完善辅助用药管理制度,提高合理用药水平。

Objective： To investigate the rules and characteristics of adjuvant drugs in our hospital, and provide references for rational use and supervise adjuvant drugs in clinic. Methods： By retrospective method, the consumption sum, frequency of drug use, defned daily cost （DDC）, B/A of the top 10 adjuvant drugs in the list of consumption sum and DDDs from 2014 – 2016 were analyzed. At the same time, the prescription/medical records from hospital management information were retrospected and the relevant contents of adjuvant drugs were evaluated. Results： During 2014 – 2016, the consumption sum of adjuvant drugs increased from 20 856 100 yuan to 21 316 200 yuan. Its proportion in total consumption sum increased from 17.48% to 18.95%. The varieties of the top 10 adjuvant drugs in the list of consumption sum included drugs for activating blood circulation to dissipate blood stasis, free radical scavenger, nerve nutrition and other types drugs. The varieties of the top 10 adjuvant drugs in the list of DDDs included drugs for activating blood circulation to dissipate blood stasis, nerve nutrition, free radical scavenger, adjuvant drugs for anticancer and other types drugs. In terms of DDC, the main dosage form was injection; B/A of these drugs was poor. Free radical scavenger, drugs for activating blood circulation to dissipate blood stasis and other types drugs ranked the top 3 in respect of unreasonable use of drugs. Conclusion： The hospital should complete the management system of adjuvant drugs and improve the level of rational drug use.
