解毒化瘀汤治疗寻常型银屑病(血瘀热结证)临床效果及对血清中TNF-α、CRP、IL-8的影响

Therapeutic Efficacy of Jiedu Huayu Decoction in Treating Psoriasis Vulgaris and the Influence of TNF-α, CRP and IL-8 Levels

目的研究解毒化瘀汤治疗寻常型银屑病血瘀热结证患者的疗效及对血清肿瘤坏死因子α(Tumor necrosis factorα,TNF-α)、C反应蛋白(C-reactionprotein,CRP)以及白细胞介素-8(Interleukin-8,IL-8)水平的影响。方法选用2014年6月—2016年5月我院皮肤科收治的寻常型银屑病血瘀热结证患者80例作为研究对象,将这些患者随机分为试验组与对照组,每组患者40例。试验组选用自拟方解毒化瘀汤加减进行内服治疗,对照组口服复方青黛胶囊,2周复诊1次,4周为1个疗程,共计观察2个疗程。计算2组患者治疗总有效率,通过比较2组患者治疗前后银屑病皮损面积和严重程度指数(Psoriasis area and severity index,PASI)来观察2组患者的临床疗效,检测2组患者TNF-α、CRP以及IL-8水平的变化。结果试验组治疗的总有效率为86.7%,显著高于对照组的77.5%,差异有统计学意义(P<0.05);2组患者治疗后PASI积分以及血清中的TNF-α、CRP、IL-8水平均明显低于治疗前,差异有统计学意义(P<0.05);试验组治疗后PASI积分为(7.68±2.13)分,低于对照组(12.86±4.88)分,差异有统计学意义(P<0.05);试验组TNF-α治疗后水平为(24.79±3.83)pg/m L,低于对照组TNF-α治疗后水平(30.82±4.85)pg/m L,差异有统计学意义(P<0.05)。结论解毒化瘀汤治疗寻常型银屑病患者疗效确切,复发率低,其作用机制可能与降低血清中TNF-α、CRP以及IL-8含量有关。

Objective In order to observe the clinical curative of Jiedu Huayu decoction in treating psoriasis vulgaris andinfluence of TNF- , CRP and IL-8 levels. Methods A total of 80 patients with psoriasis vulgaris, collected from June 2014to May 2016 in our hospital, were randomly divided into two groups, and 40 cases in each group. The observation group wastreated by Jiedu Huayu decoction orally, the control group was treated with compound indigo naturalis capsule. The two groupswere observed for eight weeks. Total efficacy, PASI scores and the level of TNF- , CRP and IL-8 levels were determined.Results The total efficacy of treatment group 86.7 was significantly higher than that of control group 77.5 , P 0.05 .The PASI scores and the level of TNF- , CRP and IL-8 was significantly reduced after treatment P 0.05 . PASI score ofobservation group 7.68 2.13 was significantly lower than that of control group 12.86 4.88, P 0.05 . After the treatment,TNF- level was 24.79 3.83 pg/mL significantly lower than that of control group [30.82 4.85 pg/mL, P 0.05 ] .Conclusion Jiedu Huayu decoction is effective for treating psoriasis vulgaris, and its mechanism might be related todecrease serum TNF- , CRP and IL-8 levels.
