观察新生儿重症肺炎并呼吸衰竭经早期持续正压通气呼吸机治疗效果 被引量:8

To Observe the Effect of Early Continuous Positive Airway Pressure Ventilator in the Treatment of Neonatal Severe Pneumonia and Respiratory Failure

摘要 目的:探讨新生儿重症肺炎并呼吸衰竭患儿接受早期呼吸机持续正压通气治疗的临床应用效果。方法:选取本院于2009年1月~2015年12月期间收治的214例新生儿重症肺炎并呼吸衰竭患儿作为本次研究对象,经随机数字表法将其分为研究组(n=107)、对照组(n=107)。对照组在常规治疗基础上加用吸氧治疗补充机体氧容量,研究组在常规治疗基础上提供早期持续正压通气(经鼻)。结果:研究组发绀、呼吸困难、肺部湿罗音等症状消失时间均显著少于对照组(P<0.05);经相应方案治疗后研究组新生儿重症肺炎并呼吸衰竭患儿p H、Pa O2上升幅度以及Pa CO2下降幅度均优于对照组(P<0.05)。结论:早期经呼吸机提供持续正压通气可显著提高新生儿重症肺炎并呼吸衰竭临床疗效,对保障患儿生活质量、生命安全具有重要的临床意义。 Objective: To investigate the clinical efect of early positive airway pressure ventilation in neonates with severe pneumonia and respiratory failure. Methods: Methods 214 cases of neonatal severe pneumonia and respiratory failure admitted to our hospital from January 2009 to December 2015 were selected as the subjects of this study. They were divided into study group (n=107) and control group (n=107) by random number table method. On the basis of conventional treatment, the control group was treated with oxygen inhalation to supplement the body oxygen capacity. The study group received early continuous positive pressure ventilation (Jing Bi) on the basis of conventional treatment. Results: In the study group, cyanosis, dyspnea, pulmonary rales disappeared time was signifcantly less than the control group, comparison of the data P〈0.05; after the scheme of the research group after treatment of severe pneumonia in neonates with respiratory failure in children with pH, PaO2 and PaCO2 increased the rate of decline was better than the control group (P〈0.05). Conclusion: Early positive pressure ventilation through ventilator can signifcantly improve the clinical efcacy of neonatal severe pneumonia and respiratory failure, and has important clinical signifcance for the protection of children?s quality of life and life safety.
