摘要
目的探讨液基细胞学技术(thinprep cytologic test,TCT)联合二代杂交捕获技术(Hybird capture,HC-2)检测高危型HPV感染在宫颈病变筛查中的价值。方法以2016年1—12月来我院就诊的481名可疑宫颈异常者行TCT及HC-2检查,结果阳性的患者行阴道镜取组织病理学检查。结果 481名检查者,TCT检查异常者81例(16.84%),HPA-DNA阳性例数为139(28.9%),对以上二者出现阳性者行病理学检查结果异常者有63例(13.09%)。TCT、HPA-DNA单独及联合检出率分别为66.67%、75.34%及90.47%,差异有统计学意义(P<0.05)。TCT联合HPV-DNA检测,敏感度、特异度、阳性预测值及阴性预测值均高于单独应用。结论 TCT和HPV-DNA均可应用于宫颈病变的筛查,二者联合检测,对宫颈病变诊断的敏感性、特异性、阳性预测值及阴性预测值均提高。
Objective To explore the signifcance of TCT and high types of HPV-DNA testing in the screening of cervical lesion. Methods 481 cases with abnormal cervical lesion were examined for TCT and HC-2 test, and the patients with positive results used histopathological examination. Results 81 cases were positive in TCT (16.84%), 139 (28.9%) cases were in high types of HPA-DNA, and the patients with positive results used histopathological examination, 63 cases were positive. Using TCT, HPA-DNA alone and combined each other, the detection rates was 66.67%, 75.34%, 90.47% respectively, the difference was statistically significant (P〈0.05). The sensitivity, specifcity, positive predictive value and negative predictive values of TCT combined with HPV-DNA detection were higher than used alone. Conclusion TCT and HPV-DNA test are the practical methods of screening cervical lesion, but TCT combined with HPV-DNA test can increase the diagnosis of sensitivity, specifcity, positive predictive value and negative predictive value.
