舒芬太尼联合右美托咪啶在ICU机械通气中镇痛镇静疗效观察 被引量:1

摘要 目的探讨舒芬太尼联合右美托咪啶在ICU机械通气患者镇静镇痛效果中的临床疗效。方法将我院ICU于2016年10月—2017年8月收治的120例机械通气的患者随机分为观察组和对照组,每组60例。对照组采用舒芬太尼联合咪达唑仑镇痛镇静,观察组采用舒芬太尼联合右美托咪啶;比较两组的镇痛镇静效果、不良反应等。结果观察组的镇痛镇静达到滴定目标时间短于对照组、住ICU时间短于对照组、达到Ramsay评分及BPS评分目标值的用药剂量少于对照组、撤药后苏醒时间短于对照组,差异有统计学意义(P<0.05)。观察组的谵妄、呼吸抑制、低血压等不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论舒芬太尼联合右美托咪啶对ICU机械通气患者的镇痛镇静效果满意,起效快,且不良反应少。 Objective To investigate the analgesic and sedative effect of sufentanil combined and dexmeimidine on ICU mechanical ventilation. Methods 120 cases of ICU mechanical ventilation patients in our hospital from October 2016 to August 2017 were randomly divided into the observation group and the control group, with 60 cases in each group. The control group was treated with sufentanil and midazolam, and the observation group was treated with sufentanil and dexmedetomidine. The analgesic and sedative effects, adverse reactions of the two groups were compared. Results The time of titrating target in the observation group was shorter than that in the control group, the time of ICU stay in the observation group was shorter than that in the control group, the dosage of Ramsay score and the target value of BPS score was less than that of the control group, and the recovery time after withdrawal was shorter than that of the control group, the difference was statistically signifcant (P〈0.05). The incidence of adverse reactions such as delirium, respiratory depression and hypotension in the observation group was lower than that in the control group, and the difference was statistically signifcant (P〈0.05). Conclusion Sufentanil and dexmedetomidine are satisfactory for analgesic and sedative effects in patients with ICU mechanical ventilation, with fewer adverse reactions and rapid onset.
