老年心力衰竭患者88例临床特征分析

Analysis of clinical characteristics of 88 elderly patients with heart failure

目的探讨老年心力衰竭患者的临床特点,为其临床治疗提供帮助。方法选择我院收治的老年心力衰竭患者88例作为研究对象,分析患者的病史、病因、体检及实验室检查结果等资料,探讨诊断与治疗体会。结果诱发因素为上呼吸道感染及肺炎20例(22.73%)、快速房颤12例(13.64%)及其他;首发症状为典型心功能不全54例(61.36%)。并发神经精神症状l4例(15.91%);合并不同程度的焦虑、抑郁情绪26例(29.55%)。临床误诊10例(11.36%),实施具有针对性治疗后显效60例(68.18%),有效24例(27.27%);死亡4例(4.55%)。结论老年心力衰竭临床表现不典型,诱发因素复杂、病死率高,应认真详细采集病史进行体格检查,合理使用辅助检查手段,提高诊断正确率,实施针对性措施提高心力衰竭的治疗效果。

Objective To investigate the clinical characteristics of elderly patients with heart failure,and toprovide help for the treatment of elderly patients with heart failure. Methods Choice for nearly two years HospitalAffiliated to Nantong University of Jiangsu Province branch treatment elderly patients with heart failure 88 cases as theresearch object,analysis of the patient＇s history,etiology,physical examination and laboratory examination results,andsummarize the clinical characteristics of aged patients with heart failure,and to explore the experience of diagnosis andtreatment. Results The predisposing factors of upper respiratory tract infection and pneumonia were 20 cases（22.73%）,rapid atrial fibrillation 12 cases （13.64%）,and typical cardiac insufficiency 54 cases （61.36%） .complicated neurological symptoms were 14 （15.91%）,had different degrees of anxiety and depression 26 cases（29.55%） .Clinical misdiagnosis was 10 cases（11.36%）, Targeted treatment was effective 60 cases（68.18%）,effective24 cases （27.27%）,and died4 cases （4.55%） . Conclusion The elderly heart failure clinical manifestation andcomplicated factors,the mortality rate is high,should be carefully detailed history collection physical examination,rational use of auxiliary examination methods,to improve the rate of correct diagnosis,the implementation of specificmeasures to improve the treatment effect of heart failure.
