目标导向优化急性呼吸窘迫综合征患者机械通气策略 被引量:3

摘要 目的:探讨个体化通气策略在急性呼吸窘迫综合征患者机械通气治疗中的优越性。方法:将60例接受机械通气的ARDS患者随机分为小潮气量组(对照组)及个体化通气组(实验组),对照组患者采用小潮气量及高呼气末正压的肺保护性通气策略;实验组在对照组的基础上实时监测呼吸力学指标,指导呼吸参数的设置,实施个体化通气策略。观察两组患者治疗前及治疗后第3天、第7天三个时间段的呼吸参数、血气分析、肺损伤程度、28d病死率、机械通气及住重症医学科时间。结果:与对照组比较,实验组在机械通气3、7d后,患者的PH值及二氧化碳分压明显改善,差异有统计学意义(P<0.05);氧合指数改善明显高于对照组,差异有统计学意义(P<0.05);同时机械通气及住重症医学科时间明显低于对照组(P<0.05)。结论:实时监测呼吸力学指标制定个体化通气策略用于ARDS患者机械通气治疗,能有效促进二氧化碳的排出,改善通气效果和氧合指数,减少肺损伤的严重程度,具有更佳的临床应用价值。
