肥胖儿童血糖、血脂代谢指标特征及其和性早熟的相关性研究

Study on the characteristics of blood glucose and lipid metabolism and its correlation with precocious puberty in obese children

目的通过探讨儿童BMI指数、血糖、血脂代谢指标特征及其与性早熟的相关性,为儿童卫生保健提供依据。方法选取2016年1月~2017年10月于我院就诊的性早熟儿童,对比分析性早熟儿童(早熟组,n=80)与正常儿童(正常组,n=200)BMI指数、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、尿酸(BUA)水平,分析上述指标与Tanner分期相关性。结果早熟组儿童肥胖率高于正常组儿童(P<0.05),早熟组儿童BMI、FPG、TG、TC、LDL、HDL、BUA代谢指标均较正常组有统计学差异(P<0.05),Tanner分期与BMI、FPG、TG、TC、LDL成正相关(P<0.05)。结论儿童BMI、FPG、TG、TC、LDL与性成熟程度有关,临床工作中应切实关注肥胖儿童,特别是伴有TG、FPG、TC、LDL异常改变的肥胖儿童的性早熟问题。

Objective To explore the characteristics of BMI,blood glucose,blood lipid metabolism and their correlation with precocious puberty in children,and to provide evidence for health care of children. Methods The children of precocious puberty in our hospital from January 2016 to October 2017 were selected.The BMI,fasting plasma glucose(FPG),triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL),high density lipoprotein(HDL) and uric acid(BUA) were compared between precocious children(precocious group,n=80) and normal children(normal group,n=200).the correlation between these indexes and Tanner stage was analyzed. Results The obesity rate of precocious children was higher than that of normal children(P < 0.05).The metabolic indexes of BMI,FPG,TG,TC,LDL,HDL and BUA in the precocious group were all significantly different from those in the normal group(P < 0.05).Tanner staging was positively correlated with BMI,FPG,TG,TC and LDL(P < 0.05). Conclusion Children's BMI,FPG,TG,TC and LDL are related to the degree of sexual maturity.In clinical work,we should pay attention to the problems of precocious puberty in obese children,especially obese children with abnormal changes of TG,FPG,TC and LDL.
