骨质疏松与血清IGF-1、ET-1及MMP水平具有相关性 被引量:7

Relationship between Osteoporosis and Serum IGF-1, ET-1 and MMP Metabolism

摘要 为了探讨骨质疏松患者血清胰岛素样生长因子(IGF-1)、内皮素(ET-1)、及基质金属蛋白酶(MMP)的水平,及其临床意义。选取2016年2月至2017年5月在我院接受治疗的骨质疏松患者作为研究对象,同时选取同期在我院体检的健康成年人作为对照。观察两组研究对象血清IGF-1、ET-1及MMP水平的差异,比较两组研究对象细胞因子水平和骨密度的差别,分析骨质疏松症患者IGF-1、ET-1及MMP水平与细胞因子和骨密度的相关性。结果显示,骨质疏松组患者的IGF-1水平低于对照组(t=38.483,p〈0.001),ET-1、MMP水平高于对照组(t=-17.859;t=-21.729;p〈0.001);骨质疏松组患者的IL-18、IL-6和hs-CRP水平均高于对照组(t=-18.468;t=-14.631;t=-11.826;p〈0.001);骨质疏松组患者的腰椎、胫骨颈和Ward三角处骨密度均低于对照组(t=3.608;t=4.483;t=3.967;p〈0.001);骨质疏松症患者的IGF-1水平与IL-18、IL-6、hs-CRP水平负相关,与骨密度正相关;ET-1、MMP水平与IL-18、IL-6、hs-CRP水平正相关,与骨密度负相关。本研究表明,骨质疏松患者的IGF-1水平较低,ET-1及MMP水平较高,且与细胞因子和骨密度密切相关。 The objectives of this study was to investigate the changes of serum insulin-like growth factor(IGF-1),endothelin(ET-1) and matrix metalloproteinase(MMP) levels in patients with osteoporosis and their clinical significance. The patients with osteoporosis treated in our hospital from February 2016 to May 2017 were selected as the subjects, and the healthy adults who underwent physical examination in our hospital were selected as the control group. The difference of serum IGF-1, ET-1 and MMP levels between the two groups were observed,cytokine levels and bone mineral density of the two groups were analyzed, and the correlation analysis of bone IGF-1, ET-1 and MMP levels, cytokines and bone mineral density in patients with osteoporosis were compared. Results showed that the level of IGF-1 in the osteoporosis group was lower than that in the control group(t=38.483; P〈0.001), and the levels of ET-1 and MMP were higher than those of the control group(t=-17.859;t=-21.729; P〈0.001); the levels of IL-18, IL-6 and hs-CRP in the osteoporosis group were higher than those in the control group(t=-18.468; t=-14.631; t=-11.826; P〈0.001); the BMD of the lumbar spine, the tibial neck and the Ward triangle in the osteoporotic group were lower than those in the control group(t=3.608; t=4.483; t=3.967;P〈0.001); the level of IGF-1 in patients with osteoporosis was negatively related to the levels of IL-18, IL-6 and hs-CRP, and positively correlated with bone mineral density. The levels of ET-1 and MMP were positively related to the levels of IL-18, IL-6 and hs-CRP, and negatively related to bone mineral density. This research suggested that the level of IGF-1 in patients with osteoporosis was low, but the levels of ET-1 and MMP were higher, which were closely related to cytokines and bone mineral density.
