摘要
目的分析IL-37、IL-39在维族草药恰玛古超声热透入、肠道微生物移植(FMT)联合曲妥珠单抗治疗兔晚期胃癌及评估生存时间中的意义。方法将85只晚期胃癌腹水兔随机分为实验组(45只)和对照组(40只),在两组基础灌肠、姑息治疗相同的情况下,实验组使用维族草药恰玛古超声热透入、FMT联合曲妥珠单抗治疗,而对照组采用DDP腹腔热化疗联合曲妥珠单抗。观测两组血清及腹水IL-37、IL-39、肠鸣音恢复、肛门排气排便等胃肠功能恢复情况以及生存时间。结果治疗后,实验组前后腹围差在48 h、72 h、96 h及120 h时均大于对照组,差异均有统计学意义(P<0.05)。实验组血清及腹水IL-37、IL-39水平均明显低于对照组,差异均有统计学意义(P<0.05)。实验组肛门初次排便时间短于对照组,中位生存时间长于对照组,差异均有统计学意义(P<0.05)。结论维族草药恰玛古超声热透入、FMT联合曲妥珠单抗治疗兔晚期胃癌腹水,疗效显著,明显延长生存时间;血清及腹水IL-37、IL-39可作为兔晚期胃癌治疗效果、评估生存时间的预测指标。
Objective To investigate the significance of IL-37 and IL-39 in treatment using phonophoresis of Brassica rapa, fecal microbiota transplantation (FMT) and trastuzumab for the rabbits with advanced gastric cancer and evaluation for survival time. Methods Eighty-five rabbits with advanced gastric cancer and ascites were randomly divided into experimental group ( n = 45 ) and control group ( n = 40), Basic enemata and alleviative treatment were conducted in both groups. The experimental group also received phonophoresis of Brassica rapa and FMT combined with trastuzumab, and the control group received hyperthermic intraperitoneal chemotherapy with cisplatin combined with trastuzumab. The IL-37 and IL-39 levels in serum and ascitic fluid, the gastrointestinal function recovery ( including the recovery of bowel sound, anal exhaust and cacation) and survival time were observed in both groups. Results At the 48 h, 72 h, 96 h and 120 h after treatment, the differences of waist circumference in the experimental group were more than those in the control group(P 〈0.05). The IL-37 and IL-39 levels in serum and ascitic fluid were lower in the experimental group compared to the control group ( P 〈 0.05 ). The duration for the first anal cacation was shorter and the median survival time was longer in the experimental group compared to the control group (P 〈 0.05). Conclusion Phonophoresis of Brassica rapa and FMT combined with trastuzumab can obtain a significant efficacy for the rabbits with advanced gastric cancer and can significantly prolong the survival time. The IL-37 and IL-39 in serum and ascitic fluid can be used as indicators for predicting the therapeutic efficacy and survival time in the rabbits with advanced gastric cancer.
