温针灸治疗肾虚血瘀型精索静脉曲张临床观察

Clinical observation warm acupuncture and moxibustion for treatment of kidney deficiency and blood stasis type varicocele

目的:验证温针灸治疗肾虚血瘀型精索静脉曲张的临床疗效。方法:将70例患者随机分为温针灸组(35例)及药物组(35例)。温针灸组以关元、气海、足三里、三阴交为主穴,针刺得气后于主穴处各施灸2壮,每日治疗1次;药物组口服桂枝茯苓胶囊,每次3粒,每日3次。观察两组患者治疗前后彩色多普勒成像、精液质量分析、临床有效率及复发率等情况。结果:与治疗前相比,两组患者治疗后彩色多普勒下精索静脉最大内径(D_R)变窄、反流持续时间(T_R)减少,两组超声分级均下降(均P<0.01);且温针灸组优于药物组;精液质量均较治疗前提高,且疗效相似;临床有效率温针灸组为91.4%,药物组为88.6%,组间差异无统计学意义(P>0.05);复发率温针灸组为14.2%,药物组为47.1%(P<0.05)。结论:温针灸疗法治疗肾虚血瘀型精索静脉曲张近、远期疗效较佳。

Objectives： To verify the clinical effect of warm acupuncture and moxibustion for treatment of kidney deficiency and blood stasis type varicocele（ VC）. Methods： 70 patients were randomly divided into two groups： acupuncture group（ n = 35） and drug group（ n = 35）. With the warming acupuncture group,Guanyuan,Qihai,Zusanli and Sanyinjiao acupoints were as the main points for moxibustion,once daily; drug group were treated with Guizhifuling capsule,three capsules each time,three times a day. Before and after treatment,color Doppler imaging,semen quality analysis,clinical efficacy and recurrence rate were observed before and after treatment in two groups. Results： After treatment,in the two groups,maximal internal spermatic vein diameter（ DR）narrowed the reflux duration（ TR） reduced,and ultrasound grading decreased（ all Ps〈 0. 01）; and the above indexes of acupuncture group were better than these of drug group; Semen quality of two groups was improved and the effects were similar; the clinical efficiency of acupuncture group was 91. 4% and it was 88. 6% in the drug group.There was no significant difference between the two groups（ P〉 0. 05）; the recurrence rate of acupuncture group was 14. 2% and it was 47. 1% in drug group（ P 〈0. 05）. Conclusion： The curative effect of warm acupuncture and moxibustion on the treatment of kidney deficiency and blood stasis type varicocele is better,with good long-term curative result.
