胸壁皮脂腺癌合并大疱性类天疱疮1例

A Case of Sebaceous Carcinoma on the Chest Wall Accompanied by Bullous Pemphigoid

患者男,79岁。右胸壁不规则粗糙斑块1年余,并全身张力性水疱1个月余。病理活检:肿瘤细胞呈分叶状分布,其内可见基底样细胞及皮脂腺样细胞,肿瘤细胞可见明显异型性;表皮下水疱,表皮下及真皮浅层慢性炎细胞伴嗜酸性粒细胞浸润。免疫组织化学示:EMA、AR阳性,Ber-Ep4阴性。诊断:皮脂腺癌;大疱性类天疱疮。

A 79-year-old male presented with irregular rough plaques on the right chest wall for more than one year, and tension blister on whole body for more than one month. Histopathology showed that the tumor cells were lobulated, consisting of basaloid cells and sebaceous gland cells. Tumor cells were atypia. In addition to subepidermal blisters, chronic inflammatory cells and eosinophils presented in both the epidermis and the dermis. Immunohistochemistry demonstrated positive for EMA and AR, but negative for Ber-Ep4. The diagnosis was sebaceous carcinoma complicated with bullous pemphigoid.
