80岁以上高龄心绞痛患者冠脉病变情况及危险因素分析

摘要 目的探讨80岁以上高龄心绞痛患者冠脉造影情况及冠脉病变相关危险因素。方法回顾性分析郑州人民医院2015年1月至2016年12月行冠脉造影术的80岁以上心绞痛患者的临床资料。按冠脉造影结果分为非冠心病组、单支病变组、双支病变组、三支病变组。采用SPSS 17.0软件对冠脉病变危险因素进行分析。结果 117例80岁以上高龄心绞痛患者中有17例冠脉正常,52例三支病变。高密度脂蛋白在非冠心病组明显高于三支病变组,差异具有统计学意义(P<0.05)。三支病变组中空腹血糖、糖化血红蛋白、血尿酸、收缩压等明显高于非冠心病组,差异具有统计学意义(P<0.05)。结论收缩压、空腹血糖、糖化血红蛋白是高龄冠脉多支病变的危险因素,高密度脂蛋白与冠脉多支病变呈负相关。 Objective To investigate the coronary angiography and the risk factors of coronary lesions in patients over 80 years old diagnosed with angina pectoris. Methods We retrospectively reviewed the clinical data of patients over 80 years old diagnosed with angina pectoris and received coronary angiogram in people's Hospital of Zhengzhou from January 2015 to December2016. Patients were divided into normal vessel group,single vessel lesion group,double vessel lesions group and three vessel lesions group according to the results of coronary angiography. The risk factors of coronary artery lesions were analyzed by SPSS 17. 0 software. Results 117 patients were retrospectively studied. Normal vessel group had 17 patients and three vessel lesions group had 52 patients. High density lipoprotein in normal group was significantly higher than three vessel lesions group and the difference was statistically significant( P〈0. 05). The fasting blood glucose,glycosylated hemoglobin,urine acid and systolic blood pressure in three vessel lesions group were significantly higher than the normal group and the difference was statistically significant( P〈0. 05). Conclusion Systolic blood pressure,fasting blood glucose and glycosylated hemoglobin were risk factors for coronary artery multivessel lesions in elderly patients. High density lipoprotein was negatively correlated with multivessel coronary artery lesions.
