健康体检人群不同肥胖指标与糖尿病及空腹血糖受损的关系研究

Relationship between obesity indexes and diabetes,impaired fasting glucose in health physical examination population

目的探讨不同肥胖指标与糖尿病及IFG的关系。方法采用横断面研究方法,以开滦集团有限责任公司2006~2007年度健康体检且与本研究相关体检数据完整的87757名对象作为观察队列,分别以BMI、WC、WHR及腰围身高比值(WHtR)分组,比较各组FPG水平、糖尿病和IFG的检出率。多因素Logistic回归分析影响糖尿病和IFG的因素。结果不同性别组FPG水平、糖尿病和IFG的检出率随BMI、WC、WHR和WHtR值增加而逐渐升高。校正年龄、血压和血脂因素后,Logistic回归分析显示,男性以WHtR预测糖尿病风险的OR值最高(1.649),女性以WC的OR值最高(2.495),BMI的OR值在两性中均最低(1.396/1.429);男性以BMI预测IFG风险的OR值最高(1.208),女性以WC的OR值最高(1.348)。结论BMI、WC、WHR和WHtR值越大,糖尿病和IFG的检出率越高,WC、WHR和WHtR对糖尿病风险的预测优于BMI。

Objective To explore the relationship between different obesity indexes and diabetes, impaired fasting glucose（IFG）. Methods A total of 87,757 subjects with complete medical data who underwent healthy examination from 2006 to 2007 in Kai Iman Group Ltd were enrolled in this cross sectional study. All the subjects were divided into groups according to their body mass index （BMI）, waist circumference（WC） ,waiskhip ratio（WHR） and waist-to-height ratio（WHtR）. The level of fasting plasma glucose（FPG） and the detection rate of diabetes and IFG were compared in each group. Multiple logistic regression analysis was used to analyze the influencing factor for diabetes and IFG. Results The level of FPG,the detection rate of diabetes and IFG increased along with the increase of BMI, WC, WHR and WHtR in different gender groups. Logistic regression analysis showed that WHtR（OR= 1. 649） in men and WC（OR=2. 495） in women were the most remarkable predictors for diabetes after adjustment for age, blood pressure and blood lipid. The ORs of BMI were the lowest in both men and women groups （1. 396/ 1. 429）. BMI（OR=1. 208） in men and WC （OR=1. 348） in women were the most remarkable predictors for IFG. Conclusion The detection rate of diabetes and IFG is progressively increased with increasing BMI,WC, WHR and WHtR. The prediction power of WC, WHR and WHtR for diabetes was prior to BMI.
