基于责任助产模式的优质护理干预对无痛分娩孕妇母婴结局的影响

The influences of high quality nursing intervention based on responsibility midwifery pattern on maternal-infant outcome of pregnant women undergoing painless delivery

目的探讨基于责任助产模式的优质护理干预对无痛分娩孕妇母婴结局的影响。方法选取2016年1月至2017年1月实施无痛分娩孕妇126例为研究对象,采用随机数字表法将孕妇分为2组,每组63例。对照组孕妇采用常规护理,观察组孕妇采用基于责任助产模式的优质护理干预,比较2组总产程时间、产后住院时间、产后出血量、分娩方式、母婴结局与护理满意度之间的差异。结果观察组总产程时间、产后住院时间明显短于对照组,产后出血量明显低于对照组,2组比较差异有统计学意义(P<0.05)。观察组自然分娩率明显高于对照组,剖宫产率明显低于对照组,差异有统计学意义(P<0.05)。观察组胎儿宫内窘迫发生率、婴儿并发症总发生率、下肢疼痛麻木发生率与孕产妇产后并发症总发生率明显低于对照组,2组比较差异有统计学意义(P<0.05)。观察组护理满意度明显高于对照组,2组比较差异有统计学意义(P<0.05)。结论基于责任助产模式的优质护理干预有助于缩短总产程时间、产后住院时间,提高自然分娩率,降低母婴并发症发生率,改善无痛分娩孕妇的护理质量,值得临床推广应用。

Objective To observe the effects of high quality nursing intervention based on responsibility midwifery pattern on maternal-infant outcome of pregnant women undergoing painless delivery. Methods One hundred and twenty-six pregnant women who underwent painless delivery in our hospital from January 2016 to January 2017 were enrolled in the study.These pregnant women were divided into two groups according to random number table,with 63 pregnant women in each group. The pregnant women in control group were treated by routine nursing,however,the pregnant women in observation group were treated by high quality nursing intervention based on responsibility midwifery pattern. The differences of total labor time,the duration of postpartum hospitalization,the amounts of postpartum hemorrhage,delivery way,maternal-infant outcome and nursing satisfaction degree were observed and compared between the two groups. Results The total labor time,the duration of postpartum hospitalization in observation group were obviously shorter than those in control group,however,the amount of postpartum hemorrhage in observation group was significantly less than that in control group（ P〈0. 05）. The natural birth rate in observation group was obviously higher than that in control group,however,the cesarean delivery rate in observation group was significantly lower than that in control group（ P〈0. 05）. The incidence rates of fetal distress,infant complications,lower limb pain numbness and maternal postpartum complications in observation group were significantly lower than those in control group（ P〈0. 05）. Moreover the nursing satisfaction degree in observation group was significantly higher than that in control group（ P〈0. 05）. Conclusion High quality nursing intervention based on responsibility midwifery pattern is helpful to shorten the total labor time,postpartum hospitalization duration and enhance natural birth rate,reduce the incidence of the maternal-infant complications and improve nursing quality for pregnant women undergoing painless delivery,therefor,which is worth using widely in clinical practice.
