糖肾煎治疗2型糖尿肾病的临床研究

Clinical Research of Tangshenjian in The Treatment of Type 2 Diabetic Nephropathy

目的:观察中药复方糖肾煎治疗2型糖尿病肾病的临床疗效。方法:100例2型糖尿病肾病(Ⅲ、Ⅳ期)患者,按随机数字表法分为治疗组和对照组各50例,对照组在控制代谢基础上给予盐酸贝那普利片(10mg/片、1次/d、口服),治疗组在对照组基础上加用中药汤剂糖肾煎,于治疗前、治疗1疗程(3个月)、2疗程(6个月)后比较中医症状评分,检测并比较2组患者24h尿白蛋白(24h-UAlb)、血β_2微球蛋白(血β_2-MG)、尿β_2微球蛋白(尿β_2-MG)、血肌酐(SCr)、尿素氮(BUN)、血清胱抑素C(Cys C)、空腹血糖(FPG)、餐后2h血糖(2h PG)、糖化血红蛋白(HbA1C)、甘油三脂(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)等指标。结果:治疗组总有效率91.3%,明显优于对照组;与治疗前比较,2组中医症状总积分均下降,24h-UAlb、Cys C、血β_2-MG、尿β_2-MG、Hb A1C、FPG、2hPG、TC、TG、LDL-C等指标均有改善;治疗组在24h-UAlb、CysC、血β_2-MG、尿β_2-MG、HbA1C、FPG、2hPG等指标改善方面优于对照组。结论:糖肾煎能有效缓解2型糖尿病肾病患者症状,调节糖脂代谢,减少尿蛋白,延缓其病程。

Objective： To observe the clinical effect of traditional Chinese medicine compound Tangshenjian in the treatment of type 2 diabetic nephropathy. Methods： 100 cases patients（ type 2 diabetic nephropathy with phase Ⅲ and Ⅳ）were randomly assinged to the treatment group（ 50 cases） who treated by Tangshenjian combined with Benazepril tablet and the control group（ 50 cases） treated by Benazepril tablet（ 10 mg per day）,and both groups were given controling metablism.Three months for one course of treatment,a total of two courses of treatment. Traditional chinese medicine syndrome score,The 24-hour urinary protein quantity（ 24-UAlb）,blood β2 microglobulin（ β2-MG）,urine β2 microglobulin（ β2-MG）,Serum Creatinine（ SCr）,Blood urea nitrogen（ BUN）,Serum Cystatin C（ Cys C）,Fasting blood glucose（ FPG）,2 h postprandial blood glucose（ 2 hPG）,Glycosylated hemoglobin（ HbA1C）,Total Cholesterol（ TC）,Low Density Lipoprotein cholesterol（ LDL-c）,High Density Lipoprotein cholesterol（ HDL-c）,Triglyceride（ TG） and clinical effective rate of both groups were observed before and after one course treatment and two course treatment. Resulets：（ 1） The effective rate was 91. 3% in the treatment group and 71. 4% in the control group with statistical differences when compared with each other.（ 2） an evident decrease in syndrome score,24-UAlb,Cys C,blood β2-MG,urine β2-MG,Hb A1 C,FPG,2hPG,TC,TGand LDL-c after treatment of the two groups,and an significant decrease in syndrome score,24-UAlb,Cys C,blood β2-MG,urine β2-MG,Hb A1C,FPG,2hPG of the treatment group was superior to the control group. Conclusion： Tangshenjian can effectively alleviate the symptoms of type 2 diabetic nephropathy by improving glycolipid metabolism,reduce urine protein,and prolong its progress.
