术前增强CT检查最佳图像特征与老年非小细胞肺癌淋巴结转移的相关性研究

Study on the correlation between the characteristics of preoperative enhanced CT examination and the lymph node metastasis in elderly patients with non-small cell lung cancer

目的探讨术前增强CT检查最佳图像特征与老年非小细胞肺癌淋巴结转移的相关性。方法选取2010年1月至2016年12月本院行增强CT检查的老年非小细胞肺癌病人286例,其中淋巴结转移病人182例,无淋巴结转移病人104例。所有病人均进行非小细胞肺癌淋巴结转移的最佳图像特征分析。结果单因素分析发现,肿瘤的位置、肿瘤直径、增强CT值、磨玻璃影(GGO)、胸膜凹陷、肿大淋巴结、淋巴结肿大位置、淋巴结短径、淋巴结边界都是术前增强CT检查老年非小细胞肺癌淋巴结转移r最佳图像特征(P<0.05);多因素分析发现,肿瘤的位置(肺上叶)、肿瘤直径(≥30 mm)、胸膜凹陷、肿大淋巴结、淋巴结肿大位置(纵隔淋巴结肿大及肺门、纵隔淋巴结均肿大)、淋巴结短径(5~10 mm、≥10 mm)、淋巴结边界模糊都是术前增强CT检查老年非小细胞肺癌淋巴结转移最佳图像特征(P<0.05)。结论术前增强CT检查的肿瘤位置、肿瘤直径、胸膜凹陷、肿大淋巴结、淋巴结肿大位置、淋巴结短径、淋巴结边界都与老年非小细胞肺癌淋巴结转移具有相关性。

Objective To explore the correlation between the characteristics of preoperative enhanced CT examination and the lymph node metastasis in elderly patients with non-small cell lung cancer.Methods A total of 286 cases of elderly patients（including 182 lymph wole metastasis patients） with non-small cell lung cancer who underwent CT examination in our hospital from January2010 to December 2016 were selected to analyze the best image features of lymph node metastasis of nonsmall cell lung cancer.Results Univariate analysis showed that tumor location,tumor diameter,enhanced CT value,ground-glass opacity,pleural indentation,enlarged lymph nodes,lymph node location,lymph node short diameter and lymph node boundary were the best image features of preoperative enhanced CT examination for lymph node metastasis（P〈0.05）; Multivariate analysis showed that tumor location（upper lobe）,tumor diameter（≥30 mm）,pleural indentation,enlarged lymph nodes and lymph nodes position（mediastinal lymphadenopathy and hilar and mediastinal lymph nodes were enlarged lymph nodes）,lymph node short diameter（5-10 mm,≥10 mm）,fuzzy lymph node boundaries were the best image features of preoperative enhanced CT examination for non-small cell lung cancer with lymph node metastasis（P〈0.05）.Conclusions Preoperative enhanced CT examination shows that tumor location,tumor diameter,pleural indentation,enlarged lymph nodes,lymph node location,lymph node short diameter,lymph node boundaries are correlated with the lymph node metastasis in elderly patients with non-small cell lung cancer.
