糖尿病自我管理模式在糖尿病护理管理中的效果评估

目的评价糖尿病自我管理模式对2型糖尿病病人管理效果的影响。方法将226例2型糖尿病病人根据入院顺序分为干预组112例和对照组114例,干预组采用上海复旦大学公共卫生学院建立的慢性病病人自我管理项目模式,即开展6次糖尿病病人自我管理小组活动,每周1次,连续6周;对照组采用常规的糖尿病教育方案。于在管理前和管理后1年进行2次问卷调查、血压测量和实验室检测,评价干预效果。结果干预后,干预组糖化血红蛋白和血糖水平下降显著高于对照组,2组比较差异都具有统计学意义。干预后,干预组自我管理行为量表得分提高较对照组多2.32分(P<0.001)。结论糖尿病自我管理模式有助于病人的血糖控制,提高糖尿病病人自我管理能力。

Objective To evaluate the effects of the diabetes self-management program in patients with type 2 diabetes mellitus（T2DM）.Methods A total of 226 patients with T2DM were assigned into intervention group（n = 112） and control group（n = 114） according to their order of admission.The patients in the intervention group were treated with the blueprint of Shanghai Chronic Disease Self-Management Program（CDSMP） developed at Fudan University,which included six sessions scheduled on consecutive weeks.While the patients in the control group were treated with the conventional education program on diabetes.Questionnaire survey and laboratory detection were carried out before or 1 year after intervention.Results The decrease of the levels of glycosylated hemoglobin（HbA1c） and fasting blood glucose（FBG） in the intervention group were more significantly than those in control group respectively.As compare to the control group,the scale score of self-management behavior of patients in the intervention group increased more significantly（P〈0.001）.Conclusions The diabetes selfmanagement program may help the patients with T2DM control their blood glucose and improve their selfmanagement behavior ability.
