An Analysis on Related Factors Affecting Retinopathy in Type 2 Diabetes Mellitus Patients

目的:探索2型糖尿病(T2DM)患者视网膜病变的危险因素。方法:选取接受治疗的T2DM患者184例,92例发生视网膜病变患者作为观察组,观察组中46例增值性视网膜病变患者作为PDR组、46例背景型视网膜病变患者作为BDR组,未发生视网膜病变T2DM患者92例作为对照组;观察对照组和观察组患者的收缩压(SBP)、舒张压(DBP)、空腹血糖(FBG)、肌酐(CRE)及总胆固醇水平(TCHO),并对BDR组和PDR组患者的SBP、DBP、年龄、FBG、病程进行比较;采用多因素Logistic回归法分析引起T2DM视网膜病变的相关危险因素。结果:对照组和观察组比较,高血压病史、患T2DM程比较,差异有统计学意义(P<0.05);观察组FBG、CRE指标于对照组,差异有统计学意义(P<0.05);PDR组的SBP、DBP均高于BDP组,BDR组患者的年龄高于PDR组,差异有统计学意义(P<0.05);糖尿病患者病程、TCHO、CRE及FBG与T2DM患者视网膜病变密切相关,FBG和患者病程是造成T2DM患者视网膜病变的独立危险因素。结论:糖尿病患者病程、TCHO、CRE及FBG与T2DM患者视网膜病变密切相关,FBG和患者病程是造成T2DM患者视网膜病变的独立危险因素。

Objective： To investigate the risk factors affecting retinopathy in patients with type 2 dia- betes mellitus （T2DM）. Methods： A total of 184 patients with type 2 diabetes who were treated in our hospital from February 2013 to April 2016 were selected as study objects. Among them, 92 patients without retinopathy as control group （ group C）, and the other 92 with retinopathy as observation group （ group O）, of which 46 patients had proliferative retinopathy （ group PDR） , and 46 cases were suf- fering retinal pigmentosis （group BDR）. Systolic blood pressure （SBP）, diastolic blood pressure （ DBP）, fasting blood-glucose （FBG）, creatinine （ CRE）, and total cholesterol （TCHO） of patients were tested and monitored and compared among groups. SBP, DBP, patient age, FBG and disease course were compared between groups PDR and BDR. Multi-factor logistic regression analysis was a- dopted for screening related risk factors. Results： The history of coronary heart disease, smoking histo- ry, and drinking history did not differ between groups O and C （ P 〉 0.05 ）. However, the history and course of hypertension and T2DM of groups O and C differed significantly. The indexes of FBG and CRE in group O were significantly different from those in group C （ P 〈 0.05）. SBP and DBP in group PDR were higher than those in group BDR, and patients in group BDR were older than patients in group PDR. The differences were significant （ P 〈 0.05 ）. Conclusion： The course of diabetic,TCHO, CRE, and FBG are closely correlated with were independent risk factors that cause retinopathy retinopathy. Specially, FEG and diabetic course in T2DM patients.
