早期高压氧治疗对透明质酸钠凝胶注射隆鼻术后并发症的疗效分析

Effect of early hyperbaric oxygen therapy on postoperative complications of augmentation rhinoplasty with HA injection

目的观察早期应用高压氧治疗透明质酸钠凝胶(HA)注射隆鼻术后并发症的效果。方法选取2012年2月—2017年2月新疆军区总医院医学美容中心治疗HA隆鼻术后并发症患者39例作为研究对象,根据治疗方式不同分为观察组(n=20)和对照组(n=19)。对照组给予常规治疗,观察组在对照组基础上给予早期高压氧治疗,2组均治疗21 d。观察2组患者治疗的临床疗效,治疗前后患者面部瘀斑面积和视力变化以及患者满意度。结果观察组的临床治疗总有效率为85.00%、满意度为90.00%均高于对照组的63.16%、68.42%(χ2=4.562、5.054,P=0.039、0.031);观察组面部血管栓塞患者治疗7、14、21 d后面部瘀斑面积分别为(3.78±1.13)cm^2、(2.62±1.32)cm^2和(1.21±0.99)cm^2,明显小于对照组(4.52±1.16)cm^2、(3.67±1.27)cm^2、(2.39±0.91)cm^2(t=-2.119、-2.656、-4.064,P=0.043、0.034、0.021);2组视网膜中央动脉阻塞患者治疗7、14、21 d后视力比较差异无统计学意义(Z=-2.011、-2.034、-2.011,P=0.431、0.452、0.431)。结论 HA注射隆鼻术后并发症早期应用高压氧治疗有较好的疗效,能明显改善面部血管栓塞导致的面部瘀斑症状,患者满意度高,值得推广使用。

Objective To investigate the effect of early hyperbaric oxygen therapy on postoperative complications of augmentation rhinoplasty with hyaluronic acid （HA）. Methods Thirty - nine cases with postoperative complications of HA rhinoplasty in hospital from February 2012 to February 2017 were selected, and they were divided into the observation group （n = 20） and the control group （n = 19） according to the treatment methods. The control group was given routine treatment, while the observation group was given early hyperbaric oxygen treatment on the basis treatment of the control group. The 2 groups were treated for 21 days; the clinical effect facial ecchymosis area, visual acuity and patient satisfaction of two groups before and after treatment were observed. Results The total effective rate of the observation group was 85.00% , and the satisfaction rate was 90.00% , which was higher than 63.16% and 68.42% of the control group （X2 = 4. 562, X2 = 5. 054,P = 0. 039, P = 0. 031 ） ; After treatment of 7d, 14d and 21d, the facial ecchymosis area of the observation group were （3.78 ± 1.13 ） cm2, （2.62±1.32） cm2 and （ 1.21± 0.99 ） cm2 respectively, which were significantly less than those of the control group （ t = - 2.119, t = - 2. 656, t = - 4. 064 ;P = 0.043, P = 0.034, P = 0. 021 ） ; There was no significant difference in patients with central retinal artery occlusion between the observation group and the control group after treatment of 7 d, 14 d, 21 d （Z= -2.011, Z= -2.034, Z= -2.011; P--0.431, P=0.452, P=0.431）. Conclusion Early hyperbaric oxygen therapy on postoperative complications of augmentation rhinoplasty with HA injection has a good effect, it can significantly improve the symptoms of facial ecchymosis caused by facial vascular embolism, and is worthy of popularization.
