肾上腺神经鞘瘤的CT影像特点(附14例报告)

CT imaging features of adrenal schwannoma(report of 14 cases)

目的探讨肾上腺神经鞘瘤的CT表现,提高对本病的诊断水平。方法回顾性分析经手术病理证实的14例肾上腺神经鞘瘤的CT表现,分析肿瘤的数目、部位、形态、大小、边缘、囊变、钙化、密度及强化方式等特点。结果 14例肿瘤均为单发,右侧肾上腺9例,左侧肾上腺5例;肿瘤边界清晰,最长径为3.2~14.7cm,平均7.0cm;4例密度均匀,10例囊变,3例钙化,2例恶性。平扫CT值25~39HU,平均33HU;增强4例轻度均匀强化,10例轻中度不均匀强化。结论肾上腺神经鞘瘤CT表现具有一定特征性,CT对肾上腺神经鞘瘤的诊断及鉴别诊断有重要价值。

Objective To explore the CT findings and clinical features of adrenal schwannoma to improve the diagnosis. Methods The CT findings of 14 patients with adrenal schwannomas proved by operation and pathology were retrospectively ana- lyzed. We analyzed the number, size and location, shape, density, edge, cystic degeneration, calcification and the enhancement of tumors. Results These masses were all single and mainly located in the right adrenal （9/14,64.3%） , and that in the other 5 cases were in the left adrenal. The tumors, with the longest diameters ranging from 3.2 cm to 14.7 cm, mean 7.0 cm, had well-defined margin. Four masses showed homogeneous density and the other 10 were cystic-solid. 3 lesions combined with calcifi- cation. Twelve cases were benign tumors, and the other 2 cases were malignant. The densities of all the masses ranged from 25 HU to 39 HU on plain scan images, with mean CT value of 33 HU. Contrast-enhanced CT scanning showed progressive inhomoge- neous enhancement mildly and moderately in 10 patients, and others showed slightly homogeneous enhancement progressively. Conclusion Adrenal schwannoma has characteristic CT features, which have important value in diagnosis and differential diagno- sis of adrenal schwannoma.
