三维成像及H-Flow技术结合彩色多普勒超声评价胎儿宫内缺氧状况的研究 被引量:18

摘要 目的探讨三维成像及H-Flow技术结合彩色多普勒超声在评价胎儿宫内缺氧状况中的临床价值及三维血管重建技术在发现胎儿宫内缺氧原因中的优势。方法在产科常规检查项目的基础上,经彩色多普勒检测胎心、脐动脉和大脑中动脉等血流参数,指标异常者通过三维成像及H-Flow技术,观察脐带形态、走行及空间转向,寻找缺氧的原因。同时利用实时三维成像观察胎儿活动状态及颜面表情。结果彩色多普勒血流参数是诊断胎儿宫内缺氧的基本指标,脐带、羊水和胎盘因素是造成缺氧的主要原因,三维成像及H-Flow技术适用于寻找宫内缺氧的原因,超声诊断与产后符合率为100%。结论三维成像及H-Flow技术结合彩色多普勒超声可以及时准确的诊断胎儿宫内窘迫的原因和程度,是对二维超声及彩色多普勒的有效补充,为临床诊疗和选择恰当的生产方式提供重要参考依据。 Objective To explore the clinical value of three-dimensional vascular reconstruction combined with color Doppler ultrasound in the evaluation of fetal hypoxia, and to explore the advantages of three-dimensional vascular reconstruction in the de- tection of fetal intrauterine hypoxia. Methods On the basis of routine obstetric examination items, the blood flow parameters of fetal heart, umbilical artery, and middle cerebral artery were detected via color Doppler. If the indexes were abnormal, umbilical cord morphology, the running and spatial turn, and the cause of hypoxia were observed by means of three - dimensional recon- strnction technique. At the same time, we observed the status of fetal activity and facial expression by real-time three-dimensional imaging. Results The flow parameter of Color Doppler was a basic index for the diagnosis of fetal hypoxia. The factors of the um- bilical cord, placenta, and amniotic fluid was the main causes of hypoxia. The three-dimensional reconstruction technique was suitable to the search for intrauterine hypoxia, and the coincidence rate of postpartum ultrasound diagnosis was 100%. Conclu- sion Three-dimensional reconstruction technique combined with color Doppler ultrasound can timely and accurately diagnose the cause and extent of fetal distress, which is an effective supplement of two-dimensional uhrasound and color Doppler, thereby pro- viding an important basis for clinical diagnosis and selection of the appropriate delivery mode.
