预充式导管冲洗器临床效果及经济学分析

The Clinical Effectiveness and Economic Analysis of Pre-filled Saline Syringes

目的探讨预充式导管冲洗器与传统手工配液冲、封管比较的临床效果及经济学分析,为合理配置医疗卫生资源提供参考。方法检索国内外公开发表的中英文文献,进行系统综述,得到效果指标。成本分析依赖于专家咨询、招标价格、各省市医疗卫生服务公开价格和文献的二手数据。结果与传统手工配液冲、封管比较,预充式导管冲洗器的使用能降低临床患者并发症发生率、医护人员针刺伤发生率,并能减少冲、封管操作时间。通过节约并发症治疗成本使临床患者获益,但与0.9%氯化钠注射液比较时会增加冲、封管成本;节约医护人员针刺伤检查治疗成本及时间成本,使医护人员获益。结论预充式导管冲洗器具有较好的临床应用价值及经济性。

Objective To explore the clinical effectiveness and cost difference between pre-filled saline syringes and manual operation,and to provide reference for the rational allocation of medical and health resources.Methods We identified public literatures in English and Chinese to conduct a systematic review to obtain health outcomes.The cost analysis was depended on expert consultation,tender price,public price of health care service in most provinces and cities,as well as secondary data in literatures.Results Compared with manual operation,pre-filled saline syringes is a better option for patients in reducing the incidence of complications,and for medical personnel in reducing the rate of needlestick injury and operation time as well. As for patients＇ benefits,the saving of medical costs contribute the most,but the flush/sealing costs will increase when compared with saline and as for medical personnel＇ benefits,it is mainly due to the saving of needlestick injury treatment costs and time costs.Conclusion The application of pre-filled saline syringes in clinic is meaningful and economical.
