国内改良及传统悬雍垂腭咽成形术治疗阻塞性睡眠呼吸暂停低通气综合征的Meta分析

Meta-analysis between modified and traditional UPPP on treatment of obstructive sleep apnea hypoventilation syndrome in domestic

目的评价国内改良悬雍垂腭咽成形术(UPPP)治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效和并发症。方法通过检索1991—2015年中国学术期刊网全文数据库、万方数据库和维普数据库中关于改良UPPP治疗OSAHS的临床病例研究和随机对照研究文献,并进一步筛查纳入文章的参考文献,随后评价纳入文章的方法学,对符合要求的文献用RevMan5.3软件进行Meta分析。结果在疗效比较方面,7篇文献符合纳入标准,经Meta分析后结果是,两组间差异无统计学意义[OR=1.20,95%CI(0.77,1.85)];在并发症比较方面,6篇文献符合纳入标准,经Meta分析后结果是,两组间的差异有统计学意义[OR=0.15,95%CI(0.09,0.28)];改良UPPP组的并发症发生率为14.21%(27/190),传统UPPP组为41.58%(79/190),差异有统计学意义(P<0.05)。结论改良UPPP和传统UPPP治疗阻塞性睡眠呼吸暂停低通气综合征1年后的总有效率差异无统计学意义,但改良UPPP的并发症发生率比传统UPPP明显降低。

Objective To evaluate the efficacy and complications of modified UPPP treatment of obstructive sleep apnea hypopnea syndrome in domestic. Methods By retrieving 1991 to 2015 full-text database of Chinese Academic Journal Net, Wanfang database and VIP database on modified UPPP treatment of obstructive sleep apnea hypopnea syndrome RCTs syndrome （RCTs） or a clinical case study, while screening the included reference lists, methodology of included studies was evaluated, and applied RevManS. 3 software for meta-analysis in documents that met the requirements. Results In terms of efficacy com- parison, seven met the inclusion criteria. Meta analysis showed that the differences between the two groups was not statistically significant [OR=1.20, 95% CI （0.77, 1.85）]. In terms of complications comparison, six met the inclusion criteria. Meta analysis showed that the differences between the two groups was statistically significant [OR= 0.15, 95% CI （0.09, 0.28）]. Conclusion No significant difference in the total efficiency between the modified UPPP and traditional UPPP treatment of ob- structive sleep apnea syndrome after one year. But complication rate of modified UPPP was significantly lower than that of tradi- tional UPPP.
