基因芯片检测系统在结核分枝杆菌耐药性检测中的应用价值评估 被引量:12

摘要 目的评估基因芯片检测系统在结核分枝杆菌耐药性检测中的应用价值。方法选取2016年1月-12月在温州市第六人民医院涂片阳性的202例疑似结核患者痰标本进行基因芯片检查,去除液体培养实验结果为污染与阴性和非结核分枝杆菌等原因的37例,以液体药敏实验为金标准,共165例患者中分析对利福平、异烟肼和MDR(同时对异烟肼和利福平耐药)诊断的敏感度、特异性、阳性预测值和阴性预测值。结果基因芯片对利福平检测的敏感度和特异度分别为89.8%和93.1%;对异烟肼检测的敏感度和特异度分别为78.7%和96.2%,对MDR的敏感度和特异度分别为73.9%和95.0%。结论该方法简便快速,灵敏度和特异度较好,在结核分枝杆菌耐药性检测中有重要应用价值。 Objective To evaluate the application value of gene chip detection system in the detection of drug resistance of Mycobacterium tuberculosis. Methods The sputum samples of 202 suspected TB patients smeared positive from January 1 to December 31,2016 in the Sixth People’ s Hospital of Wenzhou were conducted for gene chip examination. Except 37 cases showing contamination in liquid culture test,negative or non-Mycobacteria tuberculous,and with liquid drug sensitivity test as the gold standard,the sensitivity,specificity,positive predictive value( PPV),and negative predictive value( NPV) were analyzed among 165 TB patients. Results The results showed that the sensitivity and specificity of gene chip were 89. 8% and93. 1% for rifampicin,78. 7% and 96. 2% for isoniazid,and 73. 9% and 95. 0% for MDR,respectively. Conclusion Gene chip is a simple,rapid,sensitive and specific method,and has important application value in the detection of drug resistance of Mycobacterium tuberculosis.
