胃腺癌中Ki-67、HER2、P53、CEA、HIF-1α、EGFR和VEGF的表达及意义

目的探讨胃腺癌中Ki-67、HER2、P53、CEA、HIF-1α、EGFR和VEGF的表达及各指标之间的关联。方法通过免疫组织化学法检测262例胃腺癌组织中增殖细胞相关的核抗原(Ki-67)、原癌基因人类表皮生长因子受体2(HER2)、肿瘤抑制基因P53(P53)、癌胚抗原(CEA)、低氧诱导因子-1α(HIF-1α)、表皮生长因子受体(EGFR)和血管内皮生长因子(VEGF)的表达,分析各指标之间的相关性。结果 TNM分期Ⅰ、Ⅱ期胃腺癌组织中Ki-67、EGFR阳性表达率低于Ⅱ、Ⅲ期(P<0.05);农民、无神经侵犯的胃腺癌组织中HER2阳性表达率高于从事其他职业及有神经侵犯者(P<0.05),不同分化程度和Lauren’s分型的胃腺癌组织中HER2阳性表达率差异有统计学意义(P<0.05);无神经侵犯的胃腺癌组织中P53阳性表达率高于有神经侵犯者(P<0.05);无淋巴结转移及TNM分期Ⅰ、Ⅱ期的胃腺癌组织中CEA阳性表达率低于有转移者和TNM分期Ⅱ、Ⅲ期者(P<0.05);不同Lauren’s分型的胃腺癌组织中VEGF阳性表达率差异有统计学意义,无淋巴结转移的胃腺癌组织中EGFR和VEGF阳性表达率均低于有转移者(P<0.05)。HER2与Ki-67、VEGF的表达呈正相关,CEA与P53、EGFR和VEGF表达呈正相关,EGFR和VEGF表达呈正相关(P<0.05)。结论检测Ki-67、HER2、P53、CEA、EGFR和VEGF在胃腺癌组织中的表达对胃腺癌的诊断、治疗及预后判断有一定临床价值。

Objective To investigate expression and its clinical significance of Ki-67,HER2,P53,CEA,HIF-1α,EGFR and VEGF in gastric adenocarcinoma.Methods The expression of Ki-67,HER2,P53,CEA,HIF-1α,EGFR and VEGF was detected by immunohistochemistry in gastric adenocarcinoma tissues of 262 cases.Results The expression of HER2 was statistically associated with different histological grading group,the Lauren classification,and nerve invasion in the gastric adenocarcinoma.The expression of Ki-67 was significantly correlated with TNM staging.The expression of P53 protein was significantly correlated with nerve invasion.The expression of CEA protein and VEGF protein was positively correlated with lymph node metastasis and TNM staging.The expression of VEGF protein was positively correlated with Lauren classification and lymph node metastasis.HER2 protein was positively correlated with VEGF and Ki-67 protein expression.The expression of CEA protein was positively correlated with EGFR,VEGF and P53 protein expression.The expression of EGFR protein was positively correlated with VEGF protein expression.The expression of HIF-1α was not correlated with clinicopathological features and any other immunohistochemical indexes.Conclusion There was a close association between the abnormal expression and the occurrence and progression of gastric adenocarcinoma,there were complex relationship among Ki-67,HER2,P53,CEA,EGFR and VEGF,the co-detection of these proteins might have important value for diagnosis and the estimate of prognosis of the disease.The expression of HIF-1α in gastric adenocarcinoma is less valuable in diagnosis and prognosis,and needs further study.This study provides a basis for further study on the occurrence and development of gastric adenocarcinoma.
