单人份核酸联合ELISA平行检测血液筛查方案探讨 被引量:3

摘要 目的:分析探讨单人份核酸联合ELISA平行检测血液筛查的优缺点及其适宜性,为血液检测工作提供依据。方法:采用单人份血液核酸检测(ID-NAT)与酶联免疫吸附试验(ELISA)双试剂平行检测3894份献血者血液标本。结果:3894份标本NAT初筛阳性51份(1.31%),其中ELISA双(+)12份,符合率为23.5%(12/51),另外39份ELISA阴性,占76.5%(39/51)。39份NAT(+)ELISA(-)标本复查结果,阳性15份,复查阳性率仅38.5%(15/39)。发现1例抗-HCV双(+)但NAT(-)样本,NAT复查仍阴性,送国家卫生和计划生育委员会临床检验中心验证为NAT(+)。卫生部共反馈14份HBV NAT和乙肝两对半结果,其中3份NAT阳性,3份标本中2份本站初筛、复查均阳性;1份初筛阳性,复查阴性。乙肝两对半结果仅2份五项全阴,其余至少有一项阳性,以HBe Ab和HBc Ab居多。结论:单人份核酸与ELISA平行检测有利有弊,应根据实际情况选择适宜本单位的筛查方案;采用NAT初次阳性即报废的方案可最大限度降低漏检风险;NAT与ELISA在血液检测中可互补降低输血传播风险,两种检测缺一不可。
