摘要
目的:探讨冰冻液体诱导低温治疗弥漫性轴索损伤患者的护理效果。方法:选取2012年1月~2014年1月入住我院的弥漫性轴索损伤且行低温治疗的患者46例,将其随机等分为观察组和对照组。对照组仅予常规降温方法;观察组在常规降温基础上,在低温诱导阶段加用静脉内滴注冰冻液体。观察两组患者昏迷时间、护理前后格拉斯哥昏迷评分(GCS)及日常生活能力量化评分、神经功能缺损情况、出院时并发症发生率以及平均目标体温达标时间。结果:观察组患者昏迷时间短于对照组;生活能力及神经功能缺损改善情况均优于对照组(P<0.05);平均目标体温达标时间短于对照组(P<0.05),并发症发生与对照组比较无统计学意义(P>0.05)。结论:冰冻液体诱导低温治疗弥漫性轴索损伤患者能缩短其体温达标时间及昏迷时间,改善患者的神经功能缺损情况,提高患者的生活能力,而并不增加并发症的发生率,是针对该疾病安全有效的治疗措施。
Objective: To explore the nursing effect of frozen liquid induced hypothermia in patients with diffuse axonal injury. Methods: A total of 46 patients with diffuse axonal injury treated with frozen liquid induced hypothermia in our hospital from January 2012 to January 2014 were selected and randomly divided into observation group and control group. While the control group was only treated with conventional cooling method,the observation group,on the basis of conventional cooling method,was treated with intravenous infusion of frozen liquid during the hypothermia induction stage. The following aspects between the two groups were observed: the coma time,the score of Glasgow coma scale( GCS) and the daily living ability before and after nursing,the neurological deficit,the incidence of complications at discharge and the average target time of target body temperature. Results: The coma time in observation group was shorter than that in control group; living ability and the improvement of neurological deficit were better than that of control group( P < 0. 05);the average target time of target body temperature was shorter than that of control group( P < 0. 05). There was not statistical significance in comparing complications between two groups( P > 0. 05). Conclusion: Frozen liquid induced hypothermia in patients with diffuse axonal injury can shorten their time of reaching standard body temperature and the time of coma,improve their neurological deficits and living ability without increasing the incidence of complications. Therefore,it is a safe and effective treatment for the disease.
