养老院配套医疗服务现状及需求分析——以十堰市某区为例

Analysis of the Present Situation and Demand of Supporting Medical Service in Nursing Home:A Case Study of a District in Shiyan City

目的了解十堰市养老院医疗服务现状及需求状况,为提高养老院医疗服务水平提供参考依据。方法选取十堰市4家养老院开展问卷调查与访谈,内容包括一般情况、对养老院医疗软硬件服务的需求、对养老保险医疗服务的看法等。对访谈及问卷的结果进行数据分析,采用描述性分析法、Logistics回归分析法,分别对养老院老年人基本情况、影响养老院医疗护理服务人员素质需求的因素进行分析。结果该地区养老院配套医疗服务不足,单因素分析显示,不同性别、月收入、文化程度、养老观念、医疗费用支付方式的老人对养老院医疗服务需求选择方面差异有统计学意义;Logistic模型分析显示,经济水平影响老人对养老医疗服务的选择,高龄、高水平收入、高文化程度的老年人对医疗服务要求更高。结论十堰市养老院的医疗卫生服务情况不乐观,设施服务不齐全,无法满足老人的医疗服务需求。应从养老院规范化、医养结合、医保接轨、注重心理人文关怀四大方面解决问题。

Objective To gain an insight into the current medical service and requirement situations of nursing home in Shiyan as well as to provide a reference for improving the qualities of nursing home medical service.Methods 4 nursing homes were selected for the investigation and the staffs were interviewed in depth.The questionnaire,which mainly contains the general situation,the requirement of the soft-and hardware for medical service in nursing home and the viewpoint of the endowment insurance etc.The descriptive analysis and Logistic regression analysis were used to analyze the influencing factors of the requirement of nursing home care services quality.Results The nursing homes medical services were insufficient in this district.The statistical significance was presented with single-factor analysis,which revealed that the diversities of choosing a nursing home medical service are influenced by gender,incomes,education levels,pension concepts and the payment methods of medical expense.Logistic model analysis showed that the level of economic levels could affect the choice of the elderly for health care services.Higher requirements for medical equipment service were achieved when the elderly were older,with higher incomes and education levels.Conclusion The city nursing home health service situation is not optimistic because the facilities service is not complete and can't fully fill the needs for medical care of the elderly.Thus,the quality of medical service should be improved through standardization,combination of medical and support,medical insurance connection and humanistic concern.
