鼻腔冲洗联合口腔护理对鼻咽癌放疗鼻窦炎的影响

Effect of nasal cavity irrigation combined with oral nursing on nasosinusitis in patients with nasopharyngeal carcinoma receiving radiotherapy

目的探讨鼻腔冲洗联合口腔护理对鼻咽癌放疗鼻窦炎的影响。方法选取2015年3月至2016年10月间首都医科大学附属北京世纪坛医院收治的60例鼻咽癌放疗患者,其中,2015年3月至2015年12月间收治的30例鼻咽癌放疗患者为对照组,给予常规护理。2016年1月至2016年10月间收治的30例鼻咽癌放疗患者为观察组,给予碳酸氢钠鼻腔冲洗联合综合性口腔护理。比较两组患者的鼻窦炎临床症状评分、口面部活动及生活质量。结果观察组患者护理3个月后鼻塞、头疼、嗅觉减退和鼻腔分泌物等评分,均低于对照组患者,两组组间比较,差异均有统计学意义(均P<0.05)。观察组患者张口困难、吞咽障碍和颈部活动受限发生率,均低于对照组患者,两组组间比较,差异均有统计学意义(均P<0.05)。观察组患者睡眠障碍、鼻腔部症状、鼻窦炎症状和情感障碍的评分,均低于对照组患者,两组组间比较,差异均有统计学意义(均P<0.05)。结论鼻腔冲洗联合口腔护理能减少鼻咽癌放疗鼻窦炎和口面部活动受限情况,改善患者的生活质量。

Objective To investigate the effect of nasal cavity irrigation combined with oral nursing on nasosinusitis in patients with nasopharyngeal carcinoma receiving radiotherapy. Methods Sixty patients with nasopharyngeal carcinoma receiving radiotherapy at Beijing Shijitan Hospital Affiliated to Capital Medical University from March 2015 to October 2016 were selected. Among them,30 patients who were treated from January 2016 to October 2016 were selected as the observation group and 30 patients who were treated from March 2015 to December 2015 were selected as the control group. The control group was given routine nursing. Clinical symptom score and oral and facial activities and quality of life were compared between the two groups. Results At 30 months after nursing,scores of nasal congestion,headache,hyposmia and nasal secretions were lower in the observation group than in the control group（ all P〈0. 05）. The incidence of difficulty opening mouth,dysphagia,restriction of neck activity was lower in the observation group than in the control group（ all P〈0. 05）. The scores of sleep disorders,nasal symptoms,sinusitis symptoms and disturbance of emotion were significantly in the observation group than in the control group（ all P〈0. 05）.Conclusion Nasal irrigation combined with oral nursing can reduce the activity of sinusitis and limited oral and facial movement and improve the quality of life in patients with nasopharyngeal carcinoma receiving radiotherapy.
