保乳术治疗早期乳腺癌的临床疗效及对生活质量的影响

Efficacy of breast conserving therapy and the effect on quality of life in patients with early breast cancer

目的探讨保乳术治疗早期乳腺癌患者的临床疗效及对生活质量的影响。方法选取2015年6月至2016年5月间青岛市市立医院收治的96例早期乳腺癌患者,采用随机数表法分为观察组和对照组,每组48例,观察组患者采用保乳术治疗,对照组患者采用根治性手术治疗。比较两组患者的手术情况、并发症发生率、美容效果及治疗前后生活质量。结果观察组患者手术时间和术中出血量均少于对照组患者,差异均有统计学意义(均P<0.05)。观察组术后2h和12h疼痛得分均低于对照组,差异有统计学意义(P<0.05)。两组患者的皮下积液和上肢肿胀发生率比较,差异无统计学意义(P>0.05)。观察组美容效果优良率高于对照组,差异有统计学意义(P<0.05)。观察组患者治疗后生活质量得分及总分均明显高于对照组患者,差异均有统计学意义(均P<0.05)。结论保乳术治疗早期乳腺癌患者能有效的减轻创伤,并提高美容效果和改善患者生活质量。

Objective To observe the clinical efficacy of breast conserving therapy for early breast cancer and its effect on the quality of life. Methods Ninety-six patients with early breast cancer were selected at Qingdao Municipal Hospital between June 2015 and May 2016. Using a random number table,patients were randomly divided into an observation group and a control group with 48 patients in each group.The observation group received breast conserving treatment and the control group received resection. The operation situation,incidence of complications,cosmetic effectiveness and quality of life were compared between the two groups before and after the treatment. Results The operative time and intraoperative blood loss was less in the observation group than in the control group（ all P〈0. 05）. Pain scores at 2 h and 12 h were lower in the observation group than in the control group（ P〈0. 05）. There was no significant difference in the incidence of subcutaneous effusion and upper limb swelling between the two groups（ P〈0. 05）.The scores of excellent and good cosmetic results were higher in the observation group than in the control group（ P〈0. 05）. Quality of life scores and the overall scores were higher in the observation group than in the control group（ all P〈0. 05）. Conclusion Breast conserving surgery can effectively reduce trauma,improve cosmetic outcomes and quality of life in patients with early breast cancer.
