益胃汤联合西药治疗Hp阳性消化性溃疡的临床疗效及对免疫功能的影响

Clinical efficacy and immune function of Yiweidecoction combined with western medicine in the treatment of Hp positive peptic ulcer

[目的]观察益胃汤联合西药治疗Hp阳性消化性溃疡的临床疗效及对免疫功能的影响。[方法]选择2015年6月~2017年6月我院收治的Hp阳性消化性溃疡患者104例,按照随机数字表法分为观察组(52例)和对照组(52例)。对照组给予西药三联疗法(奥美拉唑、克拉霉素、阿莫西林),观察组在对照组基础上联合中药益胃汤进行治疗,治疗时间均为4周。比较2组患者治疗前后的中医证候积分、胃肠黏膜组织病理改变评分、Hp根除率及临床疗效;检测2组患者相关免疫指标水平的变化;记录2组患者随访期间复发率。[结果]治疗后,观察组患者的中医证候积分、胃肠黏膜组织病理改变评分均明显低于对照组,差异有统计学意义(P<0.05)。治疗组Hp根除率、总有效率均明显高于对照组(P<0.05)。治疗后,观察组患者免疫指标明显优于对照组(P<0.05)。观察组复发率明显低于对照组(P<0.05)。[结论]益胃汤联合西药治疗Hp阳性消化性溃疡的临床疗效明显,可改善患者免疫功能,提高溃疡愈合质量,且复发率低,值得在临床上进一步推广使用。

[Objective]To observe the clinical efficacy and immune function of Yiwei decoction combined with western medicine in the treatment of Hp positive peptic ulcer. [Methods]A total of 104 patients with Hp positive peptic ulcer admitted to our hospital from June 2015 to June 2017 ,in accordance with the ran- dora number table,were divided into observation group（52 cases）and control group（52 cases）. The cases in control group were treated with western medicine triple therapy（omeprazole, clarithromycin and amoxicil- lin） ,while the case in observation group were treated with Yiwei decoction based on the treatment in con- trol group, all the patients were treated for 4 weeks. The TCM syndrome score, pathological changes of gastrointestinal mucosa score, Hp eradication rate and total effective rate were observed. The changes of re- lated immune indexes between the two groups were detected. The recurrence rate between the two groups were recorded. [Results]The TCM syndrome score, pathological changes of gastrointestinal mucosa score in observation group were lower than those of the control group（P〈0.05）. Hp eradication rate and total ef- fective rate in observation group were higher than those of the control group（P〈0.05）. The immune inde- xes in observation group were better than those of the control group（P〈0.05）. The recurrence rate in ob- servation group was lower than that of the control group（P〈0.05）. [Conclusion]Yiwei decoction combined with western medicine in the treatment of Hp positive peptic ulcer could achieve good clinical efficacy. Itimproves the immune function, the quality of ulcer healing and reduces recurrence rate, which is worthy of clinical application.
