改良VSD冲洗方法对感染创面的效果观察

The effect of modified VSD washing method on the wound surface was observed

目的探讨感染创面行负压封闭引流(VSD)冲洗方法。方法选择2016年1月~2017年3月因创面感染入本院骨科行负压封闭引流(VSD)的患者60例作为研究对象,其中30例纳入观察组,观察组患者给予间断夹闭VSD引流管间断快速冲洗,然后改持续缓慢冲洗。另30例作为对照组,对照组给予持续缓慢冲洗。比较两组不同冲洗方法患者创面的细菌培养结果阴性例数及需二次清创手术的例数。结果观察能够充分引流出创面分泌物及坏死组织,改善局部血运,促进肉芽组织快速生长,有效控制感染。对照组分泌物引流不充分,肉芽组织生长缓慢,感染控制不理想。结论间断夹闭VSD引流管间断快速冲洗,然后改持续缓慢冲洗,能充分引流出创面脓性分泌物及坏死组织,促进创面肉芽组织形成和创面愈合,在临床值得推广。

Objective To explore the method of VSD in the treatment of infected wound.Methods From January 2016 to March 2017 due to wound infection in our hospital department of orthopedics for VSD patients 60 cases as the research object,including 30 cases in the observation group,the patients in the observation group were given intermittent occlusion of VSD drainage tube continuous quick rinse,then slowly rinse.The other 30 cases served as control group,while the control group was given continuous slow rinse.The results of two groups of patients with different irrigation methods were compared with the negative results of bacterial culture and the number of patients who needed two debridement.Results The results show that it can fully induce the outflow of pus,improve local blood circulation,promote the rapid growth of granulation tissue,and effectively control the infection.In the control group,the drainage of purulent secretion was not sufficient,the granulation tissue grew slowly and the infection control was not ideal.Conclusion Intermittent clamping VSD drainage tube continuous quick rinse,then slow rinse,can fully drain the wound purulent and necrotic tissue,promote the formation of granulation tissue and wound healing,which is worthy of clinical promotion.
