经椎弓根内固定结合椎体成形术治疗老年胸腰段脊柱骨折的临床效果观察

Observation on clinical effect of transpedicular internal fixation combined with vertebroplasty in the treatment of elderly thoracolumbar spinal fractures

目的研究老年胸腰段脊柱骨折中采取经椎弓根内固定与椎体成形术联合治疗的疗效。方法 68例老年胸腰段脊柱骨折患者,根据随机数字表法分为参照组与实验组,每组34例。参照组患者予以经椎弓根内固定术治疗,实验组患者经椎弓根内固定与椎体成形术联合治疗。观察比较两组患者治疗效果。结果实验组患者治疗总有效率(97.06%)显著高于参照组(79.41%),差异有统计学意义(P<0.05)。治疗前两组患者椎体后缘高度压缩率、椎体前缘高度压缩率及脊柱后突Cobb’s角比较差异无统计学意义(P>0.05)。治疗后,两组患者椎体后缘高度压缩率、椎体前缘高度压缩率及脊柱后突Cobb’s角比较,差异有统计学意义(P<0.05)。结论将经椎弓根内固定与椎体成形术联合应用于老年胸腰段脊柱骨折效果显著,可在临床应用推广。

Objective To study the efficacy of transpedicular internal fixation combined with vertebroplasty in the treatment of elderly thoracolumbar spinal fractures. Methods A total of 68 elderly patients with thoracolumbar spinal fractures were divided by random number table method into control group and experimental group, with 34 cases in each group. The control group received transpedicular internal fixation, and the experimental group received transpedicular internal fixation combined with vertebroplasty. The treatment effect in two groups was observed. Results The experimental group had obviously higher total treatment effective rate （97.06%）than the control group （79.41%）, and the difference was statistically significant （P〈0.05）. Before treatment, both groups had no statistically significant difference in high compression rate of vertebral body posterior margin, high compression rate of vertebral body anterior margin and Cobb＇ s angle of kyphosis （P〉0.05）. After treatment, both groups had statistically significant difference in high compression rate of vertebral body posterior margin, high compression rate of vertebral body anterior margin and Cobb＇ s angle of kyphosis （P〈0.05）. Conclusion Combination of transpedicular internal fixation and vertebroplasty shows remarkable effect in elderly thoracolumbar spinal fractures, and it can be applied and promoted in clinic.
