摘要
目的研究二维彩色多普勒超声(2D-CDU)联合血清CA125、CA19-9水平检测对卵巢癌患者诊断效能的影响。方法选取我院2015年2月~2017年5月卵巢肿瘤患者106例为观察组,其中良性卵巢肿瘤51例为A组,恶性卵巢肿瘤55例为B组,另选同期健康体检者102例为对照组。三组均行2D-CDU及血清CA125、CA19-9水平检测。比较三组血清CA125、CA19-9水平,分析2D-CDU、血清CA125、CA19-9水平单一检测及联合检测卵巢癌的诊断价值[诊断准确率(ACC)、灵敏度(SEN)、特异度(SPE)]。结果经单因素方差分析,三组CA125、CA19-9水平比较,差异有统计学意义(P<0.05);两两比较,A组CA125、CA19-9水平高于对照组,B组血清CA125、CA19-9水平高于A组,差异有统计学意义(P<0.05);三者联合检测ACC、SEN(90.56%、91.76%)高于2D-CDU(82.08%、85.88%)、CA125(85.85%、87.06%)、CA19-9(40.57%、32.94%),差异有统计学意义(P<0.05);结论血清CA125、CA19-9水平在卵巢癌中异常高表达,联合2D-CDU可明确良性卵巢肿瘤与恶性卵巢肿瘤,提高诊断准确率。
Objective To study the diagnostic efficacy of two-dimensional color Doppler ultrasonography(2 D-CDU)combined with serum CA125 and CA19-9 levels detection in patients with ovarian cancer.Methods 106 cases of ovarian cancer in our hospital from February 2015 to May 2017 were selected as the observation group,including 51 cases of ovarian benign tumor as group A,55 cases of malignant ovarian tumor as group B,another 102 healthy volunteers were taken as control group.2 D-CDU and serum CA125 and CA19-9 levels were detected in the three groups.Serum CA125 and CA19-9 levels were compared among the three groups,then analyze the 2 D-CDU,serum CA125,CA19-9 levels,single detection and combined detection of ovarian cancer diagnostic value[diagnostic accuracy(ACC),sensitivity(SEN),specificity(SPE)].Results Oneway analysis of variance,the difference of CA125 and CA19-9 levels in the three groups was statistically significant(P〈0.05).After pairwise comparison,the levels of CA125 and CA19-9 in the A group were higher than those in the control group,and the serum levels of CA125 and CA19-9 in the B group were higher than those in the A group,the difference was statistically significant(P〈0.05).The combined detection of ACC and SEN(90.56%,91.76%)was higher than that of2 D-CDU(82.08%,85.88%),CA125(85.85%,87.06%)and CA19-9(40.57%,32.94%).The difference was statistically significant(P〈0.05).Conclusion The levels of serum CA125 and CA19-9 are abnormally high in ovarian cancer.The combination of 2 D-CDU can identify benign ovarian tumors and malignant ovarian tumors,and improve the accuracy of diagnosis.
