血清降钙素原对肺结核合并肺部细菌感染的诊断价值 被引量:14

Diagnostic value of PCT in serum for pulmonary tuberculosis complicated with bacterial infection

摘要 目的探讨血清降钙素原(PCT)对肺结核合并细菌感染的诊断意义。方法回顾性分析186例肺结核患者的临床资料,肺结核合并细菌感染患者42例,单纯肺结核患者144例,比较2组患者血清PCT水平,及抗菌治疗前及治疗1周后研究组(按抗菌治疗效果分为控制组及未控制组)患者血清PCT水平。结果单纯肺结核组患者血清PCT水平低于肺结核合并肺部感染组,差异有统计学意义(P<0.05);控制组与未控制组治疗前PCT水平差异无统计学意义(P>0.05),抗感染治疗后控制组患者PCT水平较治疗前明显下降,且显著低于未控制组,差异有统计学意义(P<0.05)。结论检测血清PCT对肺结核合并肺部感染具有诊断价值,动态检测能够较好地预测抗菌药物治疗的效果,对诊断及抗菌药的选择应用具有一定的临床意义。 Objective To investigate the diagnostic value of procalcitionin( PCT) in pulmonary tuberculosis complicated with bacterial infection. Methods The clinical data of 186 patients with pulmonary tuberculosis were retrospectively analyzed. Among the 42 patients with pulmonary tuberculosis complicated with bacterial infection and 144 patients with simple pulmonary tuberculosis,the levels of serum PCT between the two groups were compared,and that of research group before the anti-bacterial treatment and after the anti-bacterial treatment for 1 week also compared( they were divided into control group and uncontrolled group according to the effect of anti-bacterial). Results PCT levels in patients with simple pulmonary tuberculosis were lower than those in pulmonary tuberculosis complicated with pulmonary infection,with the difference statistically significant( P〈 0. 05). There was no statistical significance in the difference of the PCT level between the control group and the non-control group before treatment( P〉 0. 05). The PCT level was significantly lower than that before treatment and significantly lower than that of the control group,with the differences statistically significant( P 〈0. 05). Conclusion Detecting serum PCT has the diagnostic value in pulmonary tuberculosis complicated with pulmonary infection. Dynamic detection can better predict the effect of antimicrobial treatment,and has certain clinical significance for the diagnosis and choice of antimicrobial agents.
