丙泊酚复合瑞芬太尼静脉麻醉对老年骨科手术患者免疫及认知功能的影响 被引量:14

摘要 目的分析丙泊酚复合瑞芬太尼静脉麻醉对老年骨科手术患者免疫及认知功能的影响。方法选取行膝关节置换术的老年患者60例,采用随机数表法将其分为对照组与观察组,每组30例。对照组单用瑞芬太尼麻醉,观察组给予丙泊酚复合瑞芬太尼麻醉,记录两组麻醉前后免疫细胞(CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+)水平,并采用简易精神状态量表(MMSE)评价认知功能情况。结果两组术后CD_3^+、CD_4^+、CD_4^+/CD_8^+水平较术前下降,差异有统计学意义(P<0.05)。观察组CD_3^+、CD_4^+、CD_4^+/CD_8^+改善幅度小于对照组(P<0.05)。观察组术后3 h、6 h MMSE评分高于对照组(P<0.05)。结论术前麻醉对老年患者术后免疫及认知功能有一定的影响;丙泊酚复合瑞芬太尼用于老年骨科手术麻醉,能减轻免疫反应,改善术后早期认知功能。
