PDCA循环管理法在咽痰微生物标本管理中的应用

The application of PDCA cycle management on pharynx and sputum sample in microbiological lab

目的探讨PDCA循环管理法在咽痰微生物标本管理中的应用效果。方法调查分析2015年一季度795份咽痰微生物标本送检及检出情况,针对存在的问题运用PDCA循环管理法进行管理,对比分析实施PDCA循环管理法后二季度及2015年全年与2016年咽痰微生物标本送检及检出情况。结果经PDCA循环管理后,2015年二季度咽痰微生物标本送检量(845份vs.795份)、合格率(76.09%vs.68.81%)、阳性检出率(16.45%vs.10.57%)均高于一季度;2015年与2016年各季度间平行比较,2016年各季度同期标本送检量(4 416份vs.3 574份)、合格率(83.94%vs.76.58%)、阳性检出率(18.77%vs.14.16%)均高于2015年。非标准痰液在不合格标本中所占比例最大,但其所占比例连续下降。结论 PDCA循环管理法能有效提高咽痰微生物标本留取质量和阳性检出率,对实验室质量管理的持续改进具有积极影响,值得推广。

Objective To improve the quality of pharynx and sputum sample collection in microbiological lab and provide more accurate test results for the clinician through PDCA cycle management. Methods Seven hundred and nintyfive pharynx and sputum culture samples of the first quarter in 2015 were analyzed. PDCA cycle management was applied in the next quarter and after for the existing problems. The results of the first and second quarter in 2015 and the parallel quarter in 2015 and 2016 were compared. Results After PDCA cycle management, the submission quantity, qualified rate and positive rate of the pharynx and sputum samples in the second quarter of 2015 were higher than those of the first quarter. And it was the same when the parallel quarter in 2015 compared with 2016. Meanwhile, the proportion of nonstandard sputum samples was declined continuously. Conclusion The PDCA cycle management can effectively improve the quality of the pharynx and sputum culture samples and their positive rate. It has positive effect on continual improvement of microbiological lab and is worth to be popularized in clinical laboratory.
