摘要
目的应用Oculus Keratograph眼表综合分析仪辅助分析干眼患者临床体征指标与症状之间的相关性。方法收集2016年10月~2017年5月就诊于潍坊医学院附属医院眼科门诊的160例患者的160只眼。根据干眼临床诊疗专家共识(2013)选取干眼患者,检查前均取得患者书面同意并独自完成干眼问卷。并进行裂隙灯检查眼前节、泪膜破裂时间(BUT)、Schirmer I试验(无表面麻醉)、非侵入性首次泪膜破裂时间(NIKBUT)、平均泪膜破裂时间(NIBUTav)、非侵入性泪河高度(NIKTMH)、眼红分析(R-Scan)、睑板腺分析(MeiScan)。采用Spearman秩相关分析干眼患者症状与体征之间的相关性。结果患者当中眼部症状主要是干燥感(87.50%)、异物感(81.25%),干眼的主观症状评分与泪膜破裂时间(BUT)呈负相关(r=-0.320,P<0.01);与NIKTMH无明显相关性(r=-0.240,P>0.05);与NIKBUT呈负相关(r=-0.502,P<0.01);与NIBUTav呈负相关(r=-0.725,P<0.01);与Mei-Scan呈正相关(r=0.251,P<0.01);与R-Scan呈正相关(r=0.194,P<0.05)。结论在干眼患者中应用Oculus Keratograph眼表综合分析仪所检测客观体征与患者主观症状具有一定的相关性。
Objective To study the correlation beween the subjective symptoms and objective clinical signs in dry eye patients by Oculus Keratograph. Methods A total of 160 eyes of 160 patients diagnosed in weifang medical university affiliated hospital Eye Center from October 2016 to 2017 were recruited. The dry eye patients were selected according to expert consensus of dye eye clinical diagnosis and treatment( 2013),before check,all patients signed consent form and dry eye questionnaires alone. A series of relevant examinations,including anterior segment manifestation under the slit lamp,tear film break-up time,Schirmer I test( without surface anesthesia),noninvasive first break-up time,noninvasive average break-up time,on-invasive tear meniscus height,R-Scan,Mei-Scan were conducted. The correlation of dry eye signs with symptoms were evaluated by using Spearman rank correlation coefficients. Results The mainly symptoms in dry eye patients were dryness( 87. 50%),foreign body sensation( 81. 25%),the subjective symptom scores of dry eye and burst-up time( BUT) showed a negative correlation( r = 0. 320,P〈0. 01); NIKTMH( r = 0. 240,P〉0. 240); and negatively correlated with NIKBUT( r = 0. 502,P〈0. 01); and negatively correlated with NIBUTav( r = 0. 725,P〈0.01); positively related with Mei-Scan( r = 0. 251,P〈0. 01); positively correlated with R-Scan( r = 0. 194,P〈0. 05).Conclusion In dry eye patients,there are some correlation between subjective symptoms and objective signs by oculus keratograph.
