摘要
目的探讨新生儿高胆红素血症对血乳酸水平的影响。方法选择2015年1月至2016年5月浙江省金华市人民医院收治的年龄均为生后3~7天的符合入选标准的足月新生儿共273例,其中高胆红素血症组216例,对照组为同期收治的新生儿感染性肺炎病例57例,采用回顾性分析方法,进行动脉血检测乳酸和胆红素数值并进行统计分析。结果高胆红素血症组血乳酸水平明显高于对照组(t=3.28,P<0.01),差异有统计学意义;治疗后组与治疗前组相比,新生儿高胆红素血症患儿乳酸水平指标明显降低(t=2.61,P<0.05)。而轻度、中度、重度高胆红素血症组间患儿血乳酸水平差异无显著统计学意义(t轻-中度=1.36,t中-重度=1.00,t轻-重度=0.24,均P>0.05);不同日龄的高胆红素血症患儿血乳酸水平无显著异常,P>0.05;同时发现高胆红素血症患儿自身乳酸水平与胆红素水平亦无明显相关性(r=-0.0007,tr=0.0105,P>0.05)。结论高胆红素血症患儿动脉血中乳酸水平明显偏高,经治疗后血乳酸水平明显下降;但高胆红素血症患儿各自血中胆红素水平数值与乳酸水平数值之间却无明显相关性。
Objective To discuss the influence of neonatal hyperbilirubinemia on blood lactic acid level.Methods Totally 273 cases of full-term newborns(3-7 years)who were admitted in Jinhua People's Hospital from January 2015 to May 2016 were chosen,and they met the inclusion criteria,including 216 cases of newborns in hyperbilirubinemia group and 57 cases of infectious pneumonia in control group selected in the same period.Retrospective study was conducted to measure lactic acid(mmol/L)and bilirubin(mg/dL)with arterial blood.Results The blood lactic acid level of hyperbilirubinemia group was obviously higher than that of the control group with statistically significant difference(t=3.28,P〈0.01).After treatment,blood lactic acid level of the hyperbilirubinemia group decreased significantly(t=2.61,P〈0.05).However,difference in blood lactic acid level among groups of cases of mild,moderate and severe hyperbilirubinemia was not statistically significant(tmild-moderate=1.36,tmoderate-severe=1.00,tmild-severe=0.24,all P〈0.05).Meanwhile,there was no obvious difference in blood lactic acid level of hyperbilirubinemia among newborns of different age(P〈0.05),and the level of bilirubin was not obviously correlated with blood lactic acid level(r=-0.0007,tr=0.0105,P〈0.05).Conclusion Level of blood lactic acid in arterial blood of newborns with hyperbilirubinemia is obviously high,but it decreases evidently after treatment.Blood lactic acid level is not remarkably correlated with bilirubin level in themselves.
