摘要
目的:探讨对接受手术治疗的老年骨科疾病患者进行硬膜外麻醉对其术后短期认知功能的影响。方法:选取2014年3月至2017年3月期间在贵州省黔东南州锦屏县人民医院进行手术治疗的80例老年骨科疾病患者作为研究对象。采用随机数表法将其分为硬膜外组和全麻组。在手术期间,对硬膜外组40例患者进行硬膜外麻醉,对全麻组40例患者进行全身麻醉。然后对比两组患者的麻醉恢复情况(包括术毕至睁眼的时间、术毕至拔管的时间和术毕至定向力恢复的时间等)、MMSE评分和认知障碍(POCD)的发生率。结果:1)两组患者术毕至睁眼的时间、术毕至拔管的时间和术毕至定向力恢复的时间相比,差异无统计学意义(P>0.05)。2)在麻醉前、麻醉后6 h、麻醉后12 h和麻醉后72 h,两组患者的MMSE评分相比,差异无统计学意义(P>0.05)。在麻醉后24 h,硬膜外组患者的MMSE评分高于全麻组患者,差异有统计学意义(P<0.05)3)在麻醉后24 h和麻醉后72 h,两组患者POCD的发生率相比,差异无统计学意义(P>0.05)。在麻醉后6 h和麻醉后12 h,硬膜外组患者POCD的发生率均低于全麻组患者,差异有统计学意义(P<0.05)。结论:对接受手术治疗的老年骨科疾病患者进行硬膜外麻醉的安全性较高,对其术后短期认知功能的影响较小。
